The stone man disease: fibrodysplasia ossificans progressiva Fibrodysplasia ossificans progressiva (FOP), also called Münchmeyer disease or formerly myositis ossificans progressiva, is an extremely rare connective tissue disease in which fibrous connective tissue such as muscle, tendons, and ligaments turn into bone tissue. It is the only known medical condition where one organ system changes into another. It is a severe, disabling disorder with no cure or treatment. FOP is caused by a mutation of the gene ACVR1. The mutation affects the body's repair mechanism, causing fibrous tissue including muscle, tendons, and ligaments to become ossified, either spontaneously or when damaged as the result of trauma. In many cases, otherwise minor injuries can cause joints to become permanently fused as new bone forms, replacing the damaged muscle tissue. This new bone formation (known as "heterotopic ossification") eventually forms a secondary skeleton and progressively restricts the patient's ability to move. Bone formed as a result of this process is identical to "normal" bone, simply in improper locations. Circumstantial evidence suggests that the disease can cause joint degradation separate from its characteristic bone growth. Surgical removal of the extra bone growth has been shown to cause the body to "repair" the affected area with additional bone. Although the rate of bone growth may differ depending on the patient, the condition ultimately leaves sufferers immobilized as new bone replaces musculature and fuses with the existing skeleton. 5/10 update: Charlie had a day with dad. He was chipper and active all day. He got a fun car ride, a little walk, some acupuncture. He had a really good poop in a parking lot lol. More progress in traction today. Getting the arches in my spine back slow and steady. It’s very intense but no clamp pain, and the discomfort stops when we stop applying the stretch (before the clamp pain would be heightened for weeks or sometimes months after attempting something like this). So my spine is starting to respond like healthy tissue should to normal interventions. We still are careful not to push into the clamp pain and have to introduce new things carefully and progress slowly. Oh! Another positive; my autonomic nervous system is starting to work better in addition to peripheral nerves and muscles coming back. InjectaBEAR. Patient in his thirties with a partial ACL tear with feelings of knee instability and pain. Sutured the ACL with the standard Bunnell technique and then into a knotless anchor in the notch. Instead of shuttling the BEAR into the notch, we morselized the BEAR and mixed with 15 cc of whole blood into a 20 cc syringe. Injected through jamshidi needle. We squeezed a small amount of the BEAR slurry onto the table, and it really does turn into a hydrogel within a couple of minutes. Imaging is a very helpful tool when working with a spondylolysis, as it can influence the plan of care and also give some insight into the prognosis. One important piece is that radiographic outcome of healing is not associated with clinical outcomes, and clinical outcomes take precedence over the radiographic outcome. We don’t need to rely on perfect radiographs, but rather how the patient is presenting in the clinic, and what their response is to the exercise interventions. We must also take into account that radiographic outcomes do not predict quality of life. Although radiographic healing (bony fusion) is not associated with clinical outcomes, it can still provide an estimation of treatment time based on the severity/grade of the stress fracture and still has some utility within rehabilitation and determining rough estimates of treatment duration.
