Making it to this point of becoming a U.S. Air Force physician was a dream of mine since high school. Now we’re down to the final week before medical school graduation! 🩺 ———————————————————————————————— In 2014, I flew out to Colorado to visit the Air Force Academy, as an aspiring high schooler. I had no idea what the journey would look like... all I knew was that I would hold on tightly to that dream of becoming a military doctor. Unbeknownst to me, I would spend the next almost decade of my life calling Colorado home and working towards that goal. 4 years at the Air Force Academy, 1 year on Active Duty as an Academy Admissions Advisor, and another 4 years of medical school later... Certainly, hard work played a role, but it’s not what I remember the most vividly when I look back upon this time. This chapter brought with it the most inspiring people, friendships, memories, and lessons. Medical school was in no way easy, but it’s truly because of the people that I was able to push through it. Hello everyone, my name is Karen Fong. I am a certified physician assistant currently working in orthopedic surgery. I graduated with my Bachelor’s in Biological Sciences, with an emphasis on Neurobiology, Physiology, and Behavior, from UC Davis. Prior to applying to PA school, I worked as a cardiology medical assistant and as an independent contractor for a cardiac defibrillator company. I attended PA school at Dominican University and graduated in December 2023. PA school was by far the most challenging years of my life, but I must say it was also the most rewarding and fulfilling experience of my entire student career. I grew not only as a healthcare provider but also as a person. The steep learning curve of didactic year and the demanding schedule of clinical rotations truly tested my ability to maintain a healthy work-life balance. However, it was during these tough times that I met some of my very best friends and learned what it takes to balance it all. Advice that I would like to share with anyone who is interested in this field is to ask yourself whether you truly want to help patients and whether you’re willing to go above and beyond for patients’ health. The highlights of my days are when patients tell me they actually feel better after a certain treatment plan. It’s moments like these that make all the sleepless nights worth every bit. They remind me that with each patient interaction, I am not only applying my medical knowledge but also making a real difference in someone’s life. These can detach and then embolize to the brain, particularly the branches of the middle cerebral artery (MCA). Step 2: Disease Diagnosis: Left Inferior and Superior MCA The patient has global aphasia (global impairment in language comprehension as well as fluency and word repetition) and right-sided upper limb weakness, which means his stroke lesion is likely to be located in the left-brain hemisphere. The cortical branches supply the brain parenchyma of the primary motor cortical areas and somatosensory cortical areas of the face, trunk, and upper limbs (excluding insular and auditory cortex). The MCA also supplies the internal capsule via the lenticulostriate arteries, which explains the diverse presentation of MCA infarcts. Specifically, he likely has had a stroke in the left inferior and left superior MCA, which supplies the lateral surface of the temporal lobe, anterolateral frontal lobe, and parietal lobe. The other choices are incorrect: ❌ Arcuate fasciculus strokes will cause a conduction aphasia that is characterized by problems with repeating what is said. However, patients have preserved fluency and comprehension. ❌ Left inferior and posterior MCA stroke can cause Wernicke (receptive) aphasia, which presents as impaired language comprehension with intact, nonsensical production of speech. ❌ Left lacunar artery strokes tend to have pure motor and/or sensory strokes. They may also have dysarthria. ❌ Left superior MCA stroke may present with Broca (expressive) aphasia, which is characterized by preserved comprehension but impaired language production. #LoveHowYouLearn
