🎯 In the lead up to #WorldNoTobaccoDay later this month, it is high time we recognise and effectively address the deadly synergy between the 2️⃣ epidemics: #tobacco use and #tuberculosis (TB). Tobacco is the single-largest preventable cause of death worldwide and TB continues to be the deadliest of infectious diseases in high burden countries (even despite the COVID-19 pandemic). A study of thousands of front-line healthcare workers across six countries found that those eating plant-based diets had 73 percent lower odds of suffering a moderate or severe course of COVID-19. When combined with pescatarians (who eat fish but no other meat), the workers still had significantly lower odds, 59 percent. The biggest difference in risk was found between those eating plant-based versus low-carb. Compared to plant-based healthcare workers, individuals eating low-carb, high-protein diets had nearly four times greater odds of suffering a serious infection. The greatly reduced risk was independent of underlying risk factors for COVID-19 mortality—such as being overweight or having heart disease, high blood pressure, or diabetes. The apparent plant-based protection went above and beyond. Harvard researchers collected data from nearly 600,000 participants and found that those scoring higher on a plant-based diet scoring system not only had a significantly lower risk of suffering a severe course of COVID-19, but also a significantly lower risk of getting infected in the first place. And, again, this was after taking comorbidities into account, alongside non-dietary risk factors, such as exercise and smoking. Why might a plant-based diet be helpful? Fiber, concentrated in whole plant foods, is the single most anti-inflammatory food component, whereas saturated fat, concentrated in meat, dairy, and junk, is the most pro-inflammatory food component. Diet-associated inflammation increased the risk of getting infected with COVID-19, suffering a severe course, and dying from the disease. A healthful plant-based diet may be a powerful tool to reduce the risk of severe COVID-19, as well as other diseases. Dutch attorney Meike Terhorst told The Defender, “There is no victory,” as the proposed IHR amendments give the WHO’s director-general “unlimited legislative and executive powers to declare a pandemic and the measures which need to be taken” — and strengthen existing powers as specified in the current IHR, ratified in 2005. Internist Dr. Meryl Nass, founder of Door to Freedom, told The Defender the two proposed instruments will still direct WHO member states to distribute vaccines and drugs and obey demands issued by the organization during a declared “public health emergency of international concern” (PHEIC). Independent journalist James Roguski has chronicled the negotiations for both the “pandemic agreement” and the IHR amendments on his Substack. In an April 18 post, he said that negotiations on the proposed “pandemic agreement” have “failed.” In a separate Substack post on April 23, Roguski wrote that the Intergovernmental Negotiating Body is “resorting to a form of negotiating blackmail,” by “attempting to pressure the WHO member nations to adopt and sign an incomplete agreement” at next month’s meeting.” According to Roguski, the “not-so-subtle threat” is that if member states do not sign the incomplete document, “they will not be able to continue to participate in the subsequent negotiations to work out the details.” “They have always wanted to reach an empty agreement in order to set up a Framework Convention and a brand new bureaucracy (the Conference of the Parties) [COP] that would be empowered to meet on a yearly basis off into the future, forever. “They know that they cannot show us the details of what they really want to do. Zur Zeit kursieren leider einige Falschinformationen, Mythen und Verschwörungstheorien rund um ein globales Pandemieabkommen, welches derzeit von allen 194 Mitgliedsländern der Weltgesundheitsorganisation WHO verhandelt wird. Das Pandemieabkommen soll Vereinbarungen zur Prävention, Vorbeugung und Reaktion auf Pandemien (die angesichts des Klimawandels definitiv kommen werden!) enthalten. Der schnelle Austausch von Daten und Informationen über den Erreger, damit schnellstmöglich entsprechende Diagnostika und Arzneimittel entwickelt werden und geeignete Maßnahmen getroffen werden können, ist eines der wichtigsten Beispiele darin. Der Pandemievertrag und die sogenannten Internationalen Gesundheitsvorschriften, die ebenfalls gerade überarbeitet werden, sollen den Mitgliedstaaten einen Leitfaden an die Hand geben. Gleichzeitig sollen erforderliche kollektive Maßnahmen der internationalen Gemeinschaft zur Bekämpfung von Pandemien verbindlich definiert werden. Ganz explizit geht es dabei nicht darum, dass die WHO im Pandemiefall die Macht hätte, bestimmen zu können, dass Menschen sich beispielsweise zwangsweise impfen lassen müssten oder Ausgehbeschränkungen erlassen könnte. Solche Mutmaßungen sind schlichtweg falsch. Auch weiterhin wird jedes einzelne Land ganz allein über dieserlei Maßnahmen bestimmen. Die WHO wird auch in Zukunft ihre Rolle allein darin haben ihren Mitgliedsstaaten beratend und empfehlend zur Seite zu stehen. Das Programm und das Budget bestimmen alle Mitgliedsstaaten jedes Jahr gemeinsam bei der Weltgesundheitsversammlung der WHO. Am Montag hat sich der Unterausschuss Globale Gesundheit in einer öffentlichen Anhörung mit dem internationalen Pandemieabkommen beschäftigt. Wie hat sich euer Bewegungsverhalten in der Corona-Pandemie verändert? Diese Frage stellten sich auch NAKO Wissenschaftler*innen des @dkfzheidelberg in einer kürzlich erschienenen Publikation. Dazu befragten sie rund 150.000 Teilnehmende der NAKO Gesundheitsstudie zur körperlichen Aktivität in dieser Zeit. Die Mehrheit der Befragten berichteten über einen deutlichen Rückgang der Bewegung und eine zunehmend sitzende Lebensweise, insbesondere bei Personen, die ins Home-Office wechselten. "It is relatively well accepted that #climatechange can affect human pathogenic diseases; however, the full extent of this risk remains poorly quantified. Here we carried out a systematic search for empirical examples about the impacts of ten climatic hazards sensitive to greenhouse gas (GHG) emissions on each known human pathogenic disease. We found that 58% (that is, 218 out of 375) of #infectiousdiseases confronted by humanity worldwide have been at some point aggravated by climatic hazards; 16% were at times diminished. Empirical cases revealed 1,006 unique pathways in which climatic hazards, via differ ent transmission types, led to pathogenic diseases. Is Nigeria adequately prepared for the next pandemic? It’s concerning how long it took to identify the mystery disease outbreak in Sokoto as Ascites. Does this not indicate that the country’s health security falls short of International Health Regulations (IHR) standards? The Sokoto State government has confirmed 35 cases of the new Ascites disease in the Isa local government area. It’s been a minute! While cases & wastewater activity have thankfully dipped, our “lowest” levels become higher and higher after each wave. For this update we’re looking at reported Texas cases, CDC wastewater data, PMC current and 4-week projection and J. Weiland’s 6-week projection. A reminder that as cases are no longer reportable by law in Texas, San Antonio/Bexar County no longer track or report cases. While transmission is relatively “low” now, both PMC & JWeiland projection models have us looking at an uptick in cases sooner than anticipated. On that note, we are still fulfilling mask requests (we’ve distributed a little over 1,700 masks since we restarted in February!) However, we are running very low on KN95s—if you can, please consider donating to our mask fund to purchase more! Second, the trans- gene can simply be inserted into the ‘plug-and-play’ replicon (e.g., the commercially available Simplicon plasmid of Merck/Sigma–Aldrich), allowing rapid application in case of emerging infectious disease outbreaks. Our kids were alone, too. They had to be *our* heroes, though I wish they hadn't. "The pandemic has also led to measurable and unmeasurable impacts on physicians, including health effects of COVID-19 infections, professional burnout, negative impacts on productivity, and stunted career development. However, one impact on healthcare professionals' lives has been less described—the impact of their COVID-19 activities on their families. Alberto Marcelin, a family medicine physician, and Jasmine Marcelin, an infectious diseases physician, were on the front lines when COVID-19 came to Omaha, Nebraska. Like many others, our family was significantly impacted by the pandemic, especially with 2 children in elementary school and no family living nearby to help. As physician parents, we cared for increasing numbers of extremely sick patients, responding to community vaccination needs, and public advocacy for health equity throughout the pandemic, while simultaneously navigating school closures and remote learning. We thought we understood what the impact of the pandemic was on our children, but we learned a lot more from them when we asked them to tell us their stories. They saw our stress and felt our absences. They heard and internalized our stories and worries about our own patients. Gio and Nate knew how close we were to it all, but what was most unexpected for us to hear was how much our children worried about whether we would come home from work because of our long hours or if we (or they) would get sick too, like our patients. Yet they expressed joy, and they were thoughtful about what they believed they and the world learned from the pandemic."
