Interprofessional placements are done! (I still have 3, 24hr OB shifts but we are not counting it). I love coming back to the hospital where I did my first clinical rotation. Very happy that I will be there for the next year as well. So what does a hospitalist midwife do? - First Assist in C-Sections - Triage assessments - Consulting with OB - Starting inductions - 2nd Midwife at a Birth - Assisting OB/Nursing/Peds if they are busy and need support - Participate in different codes We also have different programs that midwives run out of the hospital such as Family and Babies Clinic and Early Discharge Program. Which allows OB patients to receive midwifery care in the postpartum. We are on our final placement ish (next placement is OB 3 shifts not sure if we are going to count it) for the semester. We are doing hospitalist midwifery. Where we do 12 hour shifts to support community midwives. We take pages, do triage assessment, and be the 2nd midwife at births. We also assist in c-sections and help cover the L&D floor. I am happy to be back at my OG hospital. NICU placement was a blast! Lots of learning experiences with going on transports, rounds, lactation, resuscitations, observing intubations, workshops, working with so many different teams and observing UVCs! This made NRP (Neonatal Resuscitation Program) not look as scary! Jkkk still is a bit daunting šŸ¤¢ but made me feel more confident! Too often, however, amazing students have to take a detour in their education or even completely quit because of unexpected life circumstances. With the MCU Community Relief Fund, we are hoping to change that. I know I am smiling but with great sadness I finished my family practice rotation today šŸ˜¢ I had a blast in this rotation! If youā€™re in my class or talk to me regularly you know how much I went on about this placement and how much I enjoyed it. The reason why I enjoyed it was because I was able to really get to think outside of the box and make management plans that were best for my patients. I had a terrific preceptor as well, that was super supportive! I am going to miss family medicine and all the differential diagnosis it taught me. As youā€™ll read in her caption below, she was purchasing an NRP textbook and these were the options she was given when asked about her profession. ā€œMidwifeā€ is not a choice, but dentist, educator, student, office administrator, even ā€œother non-medical professionalā€ are all options. But not midwife. Not an entire group of thousands of professionals who possess and frequently use the life-saving skills taught by NRP. What does this say about how medical professions and medical-adjacent professions see midwives? In this example, it seems to say that they donā€™t see them at all. Reposted ā€¢ @bedheadbirth ā€œItā€™s the death by 1000 cuts for me. Time and time again #midwives are completely denied a seat at the table at critical points. For example, hereā€™s me trying to buy a #neonatalresuscitation textbook. NRP is mandatory knowledge for us, yet #midwife is not a drop down option. Sure, midwifery is ā€œnot the practice of medicineā€ but the absence of this job title, which is the sole care provider for neonates taking their first breaths around the globe, is pretty much reflective of the American attitude toward midwives as a whole. Likewise, when Maryland completely redesigned their birth certificate system this year, and forced us to use it, they had a chance to give us LDEMs our hard-earned licensed and legal seat. And we are back! We are started 3rd year with a bang (a week long intensive). We technically started back in July with precourse readings and exams. 3rd year is a fun year because we do many different interprofessional rotations. My schedule for the next 3 months is Labour and Delivery and I will be nurse, then Iā€™ll be doing a OB and following a doctor, and then my last placement for the semester will be at a midwifery practice. Iā€™m excited to learn from all these different professions. A midwifery side step šŸ‘£ (not stepping awayā€¦just yet!) Over the past few months Iā€™ve come to realise that the part of my job that was causing me the most anxiety and burnout was the delivery/ward based shift life: The missed breaks, The leaving late, The anxiety of not knowing where Iā€™ll be working every shift, The uncertainty that activity can change in mere seconds, Not knowing whoā€™s about to walk through the door, So whatā€™s the new plan? Along with my senior management team we have made the decision I am now going to be working a shift a week on the maternity telephone helpline and a shift in community doing a clinic day. Yes I donā€™t know whoā€™s going to call the helpline or whoā€™s going to come to my clinic but there will definitely be more certainty and predictability to my shifts. And no nightshifts! Also being away from the maternity wards themselves for a while will help massively I think. šŸ’« A message for third year student midwives šŸ’« Iā€™ve come across a lot of you now either at work or online who are currently in the same position I was in last year. Worried, nervous and anxious about still needing a lot of deliveries. So I wanted to put this out there for you all. I finished my last official intrapartum placement in 3rd year & went into consolidation at the end of the course still needing 25 deliveries. My cohort was hugely affected by covid & in 2020 I was pulled from my first ever intrapartum placement for lockdown. So I was behind on deliveries for the 3 years. But the main point of this post is to show you that I, like many others, needed what seemed like a ridiculous amount of deliveries by the end of the degree, but still got them! And you all will too. Needing 40 deliveries to qualify, it can be easy to slip in to the rut of honing in on the delivery side of things & being disappointed if the baby is delivered by other means, e.g forceps. Just remember that if this happens, youā€™ve cared for a women in labour which can involve all sorts which you wouldā€™ve learned from, and thatā€™s much more beneficial for when you qualify. The experience of labour cares for different women with different needs is invaluable. Delivering the baby is the easy bit. When you qualify & youā€™re in the room alone with a high risk labouring woman at the start of her induction for example, youā€™ll be grateful for the experience of caring for all sorts of women in labour, whether you actually caught the baby or not. So donā€™t let the stress of the amount of deliveries eat you up. I know itā€™s easier said than done, but remember that youā€™ve got your consolidation periods at the end to get the deliveries & youā€™ll be supported to get them, not only because your colleagues will want to help you but also because the new band 5s & 1st year students will be starting, so itā€™s in their best interest to help get you those deliveries! I hope this makes some of you feel a bit better about the situation. Youā€™re so close to the finish line now! I bet you all feel like time has flown! ā£If you havenā€™t already guessed it, Iā€™m trying to start a private midwifery service for the Widebay region. After 10 years as a midwife, and now starting my own little family I want to share the love/knowledge. Itā€™s a slow moving ball to get the process right, the endorsement, the admin for provider and prescriber numbers, insurances, the setting up supplies and the credentialing process with the hospital. Still a few months out, the idea is to provide antenatal care and postnatal care to augment the public system and potentially labour and birth care. Thereā€™s a lot to be said about the pay (or lack of) that midwives, nurses, doctors, paramedics (and their trainee counterparts!) working in the NHS receive. With burnout at an all time high, the cost of living spiralling upwards and now workers being villainized for finally taking a stand against years of lower than inflation pay rises, is it no wonder the NHS (in the words of the RCN) is ā€œhaemorrhagingā€ staff? Recently Iā€™ve encountered a few midwives leaving for less stressful yet similarly paid roles. And itā€™s all got me thinking.. With totaljobs.com listing the average salary of a UK delivery driver at Ā£32,500 is it okay that someone responsible for the safe delivery of parcels is paid a higher wage than someone responsible for the safe delivery of new life?* Surely there is something seriously wrong when society quietly accepts this? I hear the critics saying, ā€œThereā€™s no more money.ā€ But this cannot be true. I echo the words of @politicsjoe How the Government chooses to spend this countryā€™s money is an ethical decision. They have chosen this route. A wage needs to reflect the level of responsibility, the level of qualification and the level of skill an individual must possess. Of course Iā€™m not saying that delivery drivers are overpaid or unskilled. What Iā€™m (and many others) are saying is that clearly the salaries of NHS midwives, nurses, doctors and paramedics donā€™t reflect what is actually involved. When the salary of a delivery driver exceeds that of a midwife it follows that what society is saying is that there is more value in delivering parcels - commercial goods, than in caring for women and babies. What it is saying is that people are not worth as much as parcels. And I donā€™t think thatā€™s okay, do you? *Please forgive the play on words used when illustrating this point. Whilst the NMC still uses the term ā€œdeliveryā€ in relation to the birth of a baby, I appreciate babies are not parcels (thatā€™s my point) and they are not ā€œdeliveredā€ but rather birthed. #changeisneedednow #strikes #fairpay #equality #nhs
