🧨 Doctor You are not IMMORTAL You need to take care of your Telomeres. I have known few doctors who work without a break day and night by CHOICE doing extra shifts and boast that they havent slpt for last 28 hrs, 32 hrs etc. Even i worked like that few hours ago and used to feel proud of myself. But it was too late i understood that I am harming and abusing my own body in the name of serving others. But i was in the delusion that i am "Serving above self" This overly committed and super hero/heroine Complex and attitude is what is actually killing us as Doctors. Some of us don't want to work but we are made to work by our seniors, administration especially in PG residency and if we don't work then we are compared to our colleagues who work beyond the limit and we are tagged with a label called as 'KAAM CHOR' , escapist etc. I WANT you to understand YOU ARE MORE IMPORTANT TO YOUR SELF . Unless you are physically and mentally fit how can you serve your patients.?? It took me years to understand this simple logic. And still thousands of doctors on this planet don't get it and are working beyond their limit and untill unless this is understood by doctors at every level from systemic to organizational to departmental level to individual level nothing can ever change. Simply framing rules by the national bodies by putting work hour limit to <48 hrs etc wont do any help. Unless it is implemented with empathy by the HOD's senior PG's who need to understand the physical limits of their juniors working round the clock. I urge Doctors to RESPECT your BODY before You Respect EVERYBODY. This doesnt mean you need to be complacent or lazy or find excuses to serve patients on duty. No not at all. I am just saying if you want to BREATH IN you need to BREATH OUT too. BOTH go hand in hand. Work and Rest for healthcare professionals should be appropriate for a Healthy healthcare system to function for the optimum patient care. What do you think can be done to solve this problem. “Where do you seek supportive accountability?” One of the best parts of formal education and training is the presence of supportive accountability as you gain skills. You know who to ask when you need help learning something new or need a sounding board in learning from something that went awry. Once you leave the structure of a degree or certificate program, it can be more hit or miss when seeking supportive accountability. The “real world” is much more reliably populated with instances of punitive accountability. You know you’ll get a fine when your parking meter runs out. You are less clear on whom you can rely when you want to get better at time management as a busy professional. Supportive accountability is integral to the coaching process. Mental Health Connections, a clinical assessment service, helps connect Mass General Brigham employees, their spouses, partners and dependents with the most appropriate and accessible behavioral health treatment, both within and outside the Mass General Brigham system. Employees do not need to be enrolled in a Mass General Brigham health plan to use this service. The Mental Health Connections triage service has helped more than 500 employees and their family members. The goal is to create a low-barrier central pathway to inquire about the wide range of resources that exist and work with a clinician to determine treatment needs and preferences. Throughout my life, I have been blessed with incredible friends. Each stage of my life has brought new friends and experiences. And even though our lives have taken us in different directions, the memories and connections are unbreakable. I am so grateful for the different perspectives and knowledge each friend has brought to my life, and I am thankful for their role in shaping who I am today. Making friends was easy in school and during my medical training. There were so many shared experiences that it would have been impossible not to build deep, lasting bonds. But school and medical training don’t last forever. When they are done, we are catapulted into the real world to fend for ourselves. And, in the real world, making friends is hard. I cannot count the times I have coached other physicians on the difficulty of making friends after residency. While my job is to remain neutral, my first thought is: “I know, right?!” It wasn’t until I found coaching that I realized how much I lacked community. I had told myself a story that I didn’t need more friends and that I didn’t have time for a community. Physician coaching showed me that I was wrong on both counts. It turns out that you can make friends at any age. Who knew? The friendships and community I have built over the last several years changed the trajectory of my life. These supportive relationships showed me that I wasn’t alone and didn’t have to run away from medicine to be happy. In fact, a strong community may be one of the most essential tools to combat physician burnout. Ready to learn more? Physicians have 2x the rate of suicide compared to the general population (Medscape Physician Suicide Report 2022) The first time I was exposed to physician suicide, I was a medical student. The feelings of loss and sadness were indescribable. Every year since, sadly, I continue to hear the news of loosing my colleagues to this tragedy on a consistent basis. The loss permeates all levels of medical education and post-graduate careers. And I have personally felt utterly powerless in the face of the physician suicide epidemic. 💔300-400 physicians die by suicide yearly. That is 300-400 families loosing their loved one and hundreds of patients loosing their beloved doctors. 💭Every day I have asked myself this question, what can I do? How can I help? What role can I play? Because that’s all I can do. This thought has allowed some solace… 🪷Today I am proud to announce that I have joined the Dr. Lorna Breen Heroes’ Foundation Ambassador Program @drlornabreenheroesfoundation -to solidify my unwavering commitment to physician and healthcare worker wellbeing. I have been humbled to learn about the initiatives taken by the Foundation and want to share my gratitude for all the work J. Jessica Singh, MD is a former practicing emergency medicine physician and the founder and CEO of Sukhayu Wellness and the Center for Health and Wellness Coaches. She graduated from medical school at New York Medical College and completed a four-year emergency medicine residency at Yale New Haven Hospital. Her own wellness journey began when she was a physician-in-training learning how to enhance her own health and heal from burnout. After practicing as an attending physician in the community, she was the first physician to be awarded a one-year fellowship in Physician Wellness at the Stanford University School of Medicine, where she developed expertise in the science and administration of physician wellness. After completing her fellowship, she transitioned careers and founded Sukhayu Wellness with the mission of helping individuals, communities, and organizations nurture well-being, enhance fulfillment, and mitigate burnout. She currently consults with an academic health care organization to assess clinician burnout, with the goal of empowering initiatives for systems change to mitigate it. Dr. Singh is certified in coaching through the National Board for Health and Wellness Coaching and the International Coaching Federation. Furthermore, she holds certifications in yoga teacher training, Interactive Guided Imagery, mind-body medicine, Radical Remission coaching, and Ayurvedic wellness counseling. Recognizing the expanding role of coaching in enhancing population health, Dr. Singh created the Center for Health and Wellness Coaches to support the professional development of health and wellness coaches. She is also the host of The Health and Wellness Coach Journal Podcast. Anger isn’t a topic we typically find pleasant or comfortable to discuss. We may even resist labeling ourselves as “angry.” However, anger is an emotion that many physicians frequently experience. It turns out anger is a secondary emotion. While anger may sit at the tip of the iceberg, other emotions like overwhelm, frustration, powerlessness, fear, rejection, anxiety, jealousy, depression, and shame can sit just below the surface undetected without taking a closer look. I recall the first time I overheard a group of doctors discussing their own encounters with anger, and it caught me entirely off guard. Until that moment, I had believed I was the only one struggling with anger. Anger is a prevalent emotion for physicians, and the work to uncover what underlies the anger can be transformative. As physicians, we often face numerous stressors and challenges in our professional and personal lives. Back when I was super burnt out and overwhelmed trying to do it all with 3 small kids, starting a morning routine literally changed my life. It gave me space to get what I needed before the day even started. And for last few years, I just haven’t been doing it anymore. It shows. I don’t feel as relaxed in my days and am not as focussed on my personal goals. So today I’m back to it: journal, meditate, do a bit of stretching of this old bod and then head for a run. Biggest thing that will get in the way of me doing it consistently? That ever present “just sit down and get some work done - you can relax later” voice. Every physician I’ve met has that voice. And guess what… it lies! Later is busy too. There is always work to do. So I’m coming back to taking time for me first thing in the morning and trusting everything else will fall into place. Oof. I always have grand ideas about posting all the cool things I get to do with my amazing residents and medical students, but time goes so fast. This week, our family medicine team took excellent care of lots of people. Some got well and went home, some are still in the hospital, and some passed away peacefully. We brought out the calipers to look at EKGs, dealt with a lot of poop (too much, not enough… the coffee grounds at the door is a telltale sign of poop situation!), figured out how to do trauma informed pelvic exams on regular hospital beds, dealt with failing kidneys and livers and hearts and brains and the people around the organs and the families of the people of the organs. I participated in my first honor walk, speaking of organs and people. 💔 And, I had the chance to lead a one hour conversation with our Coders, Billers, and Compliance employees about gender!!! Family medicine rocks. Easy and well-being are often mistakenly conflated, but they represent distinct concepts with vastly different implications for our lives. While “easy” suggests a lack of challenge or difficulty, well-being encompasses a holistic state of physical, mental, and emotional health and fulfillment. In our pursuit of ease, we may prioritize shortcuts, quick fixes, or temporary comforts, but these often come at the expense of our long-term well-being. Conversely, well-being frequently requires a deep commitment to practices and habits that nourish and sustain us in the long run, even if they involve challenges or discomfort. 🏃 Skipping the gym today? Easy, but it doesn’t promote well-being. 🧘 Beginning a new meditation practice? Challenging, but it absolutely fosters well-being. 🍨 Indulging in dessert? Easy, but not great for long-term well-being. 📺 Turning off the TV so you can get a good night’s sleep? Challenging, but great for well-being. Choosing ‘easy’ may offer fleeting relief or satisfaction, but it’s not a reliable contributor to our overall well-being. In contrast, the path to authentic well-being may demand effort, resilience, and perseverance, but the rewards are profound and lasting. Match day trauma can’t be ignored when we consider the trajectory of physician wellbeing. In all the talk we have about burnout and wellness in doctors, I have never once overtly heard the mention of Match Day. This particular day and week will leave crucial memories and scars for many physicians. This is a Point of Focus to address and support our bright young physicians. I was fortunate to match on my first round but some of my close friends didn’t and that didn’t make me a better physician or them less than. Fast forward to now- they are serving their communities and providing excellent patient care. Received this letter detailing I may be overcharging for the services I am providing my patients. Highmark argues I may have excessively billed (undertone: fraud) for the past 12 months when I have submitted 351 claims for $40,000. I allegedly deviated from my geographical area by 25% in submitting higher complexity billing codes. The math on this is $113/encounter. Prior to the pandemic, although I may be slightly off, the cost for a patient to walk through the doors for family medicine was approximately $75-85 per visit. That difference (+$30 to $40) is the reimbursement delta used to keep the building maintained, the lights on, supplies ordered, staff and myself paid as well as other costs I can’t name. I am a physician, I see complicated and sick patients. Even my well maintained patients are complex, often having many comorbid conditions that need to be treated. Physicians realize the cost of healthcare is exorbitant and costs need to be reduced. This, however CANNOT be at the risk of not maintaining facilities, paying staff or compensating physicians. In 2022 the CEO of Highmark was compensated $9,470,000 dollars or an average daily pay of $25,945. CEOs who advocate for their employees and lead companies through difficult times deserve to be paid fairly, but clearly the CEO of Highmark and the rest of the executives are being paid vast amounts of money and generating mind-blowing profit margins for a for-profit company entrusted to provide compensation and remuneration to keep patients healthy. Supposedly the patient’s health is at the forefront of their minds. Before I found physician coaching, I was afraid to set lofty goals. One of my biggest fears was feeling disappointed. In my heart, I knew what I wanted, but I tried not to focus too much on the goal. What if I don’t make it? It will feel awful! Even worse, what will other people think of me? When I think about disappointment, Match Day 2003 immediately comes to mind. As a native New Yorker, I was a shoo-in, right? I had done a rotation at my top pick, and one of the other hospitals was a perennial favorite for graduates of my medical school. It seemed inevitable. And then, for a number of reasons I won’t detail here, it didn’t happen. Man, that was a tough day. But don’t feel bad for me. In retrospect, it was the best possible outcome. I just didn’t know it at the time. Sometimes, your dream does not become your reality. Have you ever imagined what your life could or should be, only to learn that the thing you wanted is not an option? We all have. In that moment, what choice do you have? Who or what in your life have you been measuring yourself against? It’s time to measure backward – I have largely spent my life measuring myself against an ideal. That ideal is not even self-created but proposed by society and culture to me. And every time I compare myself to this said ideal, I fall short. Every time I fall short, I feel inadequate and then I compensate for that inadequacy by compulsive hard work, people-pleasing, and saying yes to everything. Every time I compared myself to this ideal - I told myself - I would be happy when… When I have done a fellowship. When I became an attending. When I get married. When I have children. When I have more friends. When I have the perfect body. And on and on. It’s funny that the harder I work, the farther my goals and happiness seem to get. Like the moving horizon. This is when I read The Gap and the Gain by Dan Sullivan and Benjamin Hardy. In the book, they recommend measuring our success backward. 🌱Measure where you are now from where you began. I invite you to step outside your body and observe. When you compare yourself to some ideal intensely, you start feeling physically constricted. And when you think of how far you have come, measured against your own starting points, your body will relax into the feeling and thought. Ideals - do not exist. Ideals are glorified further in our current hyperactive media-driven society. And ideals are never meant to be achieved. Like the horizon, no one ever arrives at but can look up towards. ✅The exercise for this week is: 1️⃣Think of one area of your life where you are measuring yourself against a non-existent ideal. Notice how you feel physically and mentally as you compare yourself. 2️⃣Now Measure Backwards. In this specific area of life, where did you start? As you think of the progress you have made so far, how do you feel? Write down all your wins. Thrive is the kickstart you need if you're overwhelmed and depleted. Are you trying to avoid detection? 7 ways you might be trying to hide under your impostorism. 1️⃣ Overpreparing and Hard work- If you believe everyone around you is inherently more intelligent or capable, one way to avoid detection is extraordinary hard work. The ONLY reason for your success is Herculean effort. 2️⃣  Holding Back- Never giving it your best shot. Or- Remaining silent in the face of opposing opinions. Or-Intellectual flattery of superiors. 3️⃣ Maintaining a Low or Ever-Changing Profile. Flying under the radar. Job hopping. 4️⃣ Use of Charm or Perceptiveness to Win Approval- Part of you feels inadequate but another believes you are remarkable - if only the right person validates your brilliance & helps you embrace this also. But when they do, you feel like a phony. 5️⃣ Procrastination- A way to give yourself an out. You tell yourself it’s because you work best under pressure. You wait until the last minute, then when you fail, you are hardly surprised. 6️⃣ Never Finishing- By not finishing you shield yourself from possible detection & avoid the shame of being criticized. “It’s still in progress” or “I’m dabbling”. 7️⃣ Self-sabotage- Fear of being exposed can be so anxiety-producing, that you subconsciously do things to undermine your very success. None of these coping mechanisms and behaviors truly alleviate your impostor feelings. They just try to keep you “safe”. Can we talk about the food that pops up at 2am when you are on call in the hospital??? It’s amazing how tempting that food can be when you really would prefer to be cozy in bed asleep. On-call eating was one of the biggest challenges I had to overcome in order to lose weight. And for a long time, I didn’t think I would be able to. So how do you stick to your healthy eating in the middle of the night or post call? I want to share my top 3 tools that have helped me transform my own eating on call. I teach these same tools (and give help applying them) to the members of Thrive Academy for Physicians. Tool #1: Stop Being Surprised - Being on call isn’t a surprise. Wanting to overeat on call or post-call isn’t a surprise either. When you recognize that it is a recurring pattern, you can start creating solutions that work. Tool #2: Manage the Stress Of Being On Call - How you think about your call matters. It can either add to your stress (which will trigger eating) or it can help you feel calm and in control of whatever your call looks like. Tool #3: Mentally Rehearse - It is easy to forget about these tools between now and your next call shift. Practice mentally rehearsing on your way into the hospital or the morning you start call. Bonus - this is a powerful tool that can be done on the fly - you don’t have to stop what you are doing to mentally rehearse feeling great around any food that might show up. After dedicating a significant portion of your life to a particular profession, it’s not uncommon to reach a point where you begin to lose passion for your mission despite being at the peak of your expertise. The thought of continuing down the same path for another decade or two seems daunting and uninspiring. You have a desire to explore something new, but you feel trapped. Outwardly, everything seems enviable, but internally, you yearn for something that aligns more closely with your evolving aspirations and values. How could anyone else possibly understand? You have it so good. Why would you want to try something new? It turns out this is incredibly common. It happens to physicians, attorneys, accountants, architects, and many other high-achieving professionals. Hosted our monthly narrative medicine section tonight with a poem from @maryoliversdrunkcousin and offered several prompts to write about and share. Love Lyndsey’s idea of writing poems about newspaper headlines . Physicians in attendance wrote about their vision of one year from now, lost pets, and our attempts to understand the encampments and protests happening on our college campuses. I think we all left with some thoughts, reflections, and emotions to sort through and better for it.
