Breathing exercises, or breathwork, is when we intentionally control how to breathe to experience a sense of relaxation and calm in the mind and body. Day three of supporting a labouring mama through a looooong latent phase and taking a moment to sit in the garden of the home she and her partner recently moved into to manifest their family. She is shape-shifting before my eyes into mother. Her transition is hard-fought. Traversing edges she didn’t know were there and finding resilience and determination that will be her sure-footedness and her foundation. Hearing this couple in their loving communications even as they navigate such outlandish trials uplifts me on behalf of this incoming soul, their choices and all our futures. 💡Understanding HG: A Genetic Glimpse & New Hope 💡 It's the number one question: why do up to 3% of women and pregnant individuals suffer from Hyperemesis Gravidarum (HG)? Historically, pregnancy sickness was linked to the hCG hormone, which rises in early pregnancy and continues to do so until around 10 weeks of gestation. Similarly, HG was also thought to be caused by hCG. However, it was also believed that HG was psychological, which is categorically untrue. 🧬 The actual cause of HG is in your genes. 📚 It's only very recently that the cause of HG has been researched. Good news, though! In December 2023, a groundbreaking study from the University of Cambridge further uncovered the causes of Pregnancy Sickness and Hyperemesis Gravidarum and the significance of the hormone GDF15. This study opened doors for potential treatments and new ways to manage this challenging condition either 1. by desensitising women who know they'll suffer from HG ahead of a new pregnancy or 2. by treating HG sufferers targetting the GDF15 receptors in the brain. 🔍 What are Hyperemesis symptoms ? 1️⃣ Persistent Nausea & Vomiting—This goes far beyond typical morning sickness and can last well into the second trimester and beyond, making living normally difficult or impossible. 2️⃣ Dehydration - Due to frequent vomiting, staying hydrated becomes a challenge. 3️⃣ Weight Loss - Losing weight during pregnancy due to HG is common and can be concerning. HG sufferers can easily lose 10% of their pre-pregnancy weight in a few weeks. 4️⃣ Nutrient Deficiencies - Difficulty eating or keeping food down can lead to deficiencies that may require supplementation, which is a catch-22 as HG sufferers usually can't keep supplements down. 5️⃣ Sensory Sensitivities - Heightened sensitivities to smells and tastes that can trigger or worsen nausea. 🔍 Hyperemesis Gravidarum is a spectrum illness. It is on the extreme end of the pregnancy sickness spectrum. ❌ It is not morning sickness. 🙅🏽‍♀️ It’s a serious and debilitating complication of pregnancy and if it’s not treated the health of both mom and baby are at risk. HG affects around 1-3% of pregnant women. In severe cases they may be sick more than 50 times in a day and symptoms can go on for several weeks or even months. 🗓️ 🍽️🥛 Extreme nausea and vomiting affect a woman's ability to eat or drink properly. This leads to weight loss, dehydration, and malnutrition. 😓 💭 Beyond the devastating physical symptoms, HG can also take a heavy toll on a woman’s mental health. Being housebound for so long often leaves sufferers feeling lonely and isolated, and it’s not uncommon for women with HG to become depressed. HG is also often mistaken for regular morning sickness, which is an expected and even welcomed sign of early pregnancy. But HG is definitely not regular morning sickness. Treating it as such often leaves women feeling misunderstood. It’s so important that women suffering with HG get the support they need from their friends, family and healthcare professionals. What you see as your hips isn't even reflective of the inside of your pelvis. We live in a society that heavily dictates the paths we should take, often what is deemed as the ‘norms’ and working alongside guidelines. But what if this set path doesn’t serve you. What does taking radical responsibility for your pregnancy and labour look like? 🌀Reading/podcasts - learn all there is to know about the physiology of pregnancy and birth, get to know your body, how and why is it changing. 🌀Knowing your rights. Know the difference between ‘Law’ & ‘Guidelines’. The phrase “am I allowed?” should be banished from your vocabulary. 🌀Write/draw your birth preferences, clearly and directly, No ‘please’, ‘thank you’s’, and ‘if it’s possibles’ This is your opportunity to set the tone. Your body, your baby, your story. 🌀Change your caregiver. Your midwife, consultant, gp, should respect your body autonomy, offer individualised care, & be aligned with your wishes. You can change your team if it doesn’t serve you. 🌀Understand evidence. How is the evidence being explained ratios vs %, be clear on any actual perceived risks. ⭐️ A huge amount of freedom and growth comes from radical responsibility. Every time a baby is born a mother is born too. The transition that each woman goes through in pregnancy & birth is a deeply profound experience. How women birth is part of the trajectory not only for her baby’s life path but also her own. 🥼👩‍⚕️ Did you know that membrane sweeps are a form of induction? While often seen as a gentle nudge rather than a full-on intervention, sweeps are a proactive method to encourage labour to start. ⏱️ 🗂️ Membrane sweeps have become routine. So much so that they are often expected as a standard procedure. But they are indeed a form of induction with a different name. Being such a critical intervention, you must be well-informed and take the time to think it through before giving your consent. Actually...Let's think about it for a minute... In the case of a healthy pregnancy with no medical condition justifying speed-up labour and birth: - A sweep might work if your body is almost ready for labour. But if labour was imminent anyway, why have a sweep? - Conversely, if your body isn’t gearing up for labour, a sweep will probably not work. And the whole process will most likely be uncomfortable, if not painful. This is because the cervix may still be firm, positioned at the back of the vagina and closed, without the baby's head pressing down on the cervix to release the hormones needed for birth. As a yoga teacher and doula, I’ve had the privilege of working with many new mothers in the postpartum period. It's a time of great change, both physically and emotionally. It can be a challenging time, but also a beautiful time of growth and transformation. 🌸Your body is healing: Childbirth is a major event for your body, and it will need time to heal. Depending on the type of birth you had, you may have stitches, soreness, or swelling. 🌸Yoga is good for recovery: Yoga can be a wonderful way to support your body's healing and recovery after childbirth. Gentle yoga poses can help relieve tension and pain, while also building strength and flexibility. 🌸Nutrition is key: Proper nutrition is essential for postpartum healing and recovery. It's important to focus on nutrient-dense foods that will support your body's healing process. Foods high in protein, iron, and vitamin C are particularly important, as they support tissue repair and wound healing. 🌸Rest is essential: Rest is essential for postpartum healing and recovery, and it's important to prioritise rest as much as possible. This may mean asking for help from family and friends, hiring a postpartum doula, or taking naps when your baby sleeps. It's important to be patient with your body and with yourself. Healing takes time, and it's important to give yourself grace and space to adjust to the changes that come with motherhood. Remember that you are doing the best you can. 🌼 Things Rarely Are What They Seem 🌼 These pictures might look like tranquil moments of mother-daughter bonding—and it is, but it's also just a snapshot, a brief pause in the relentless challenge of early motherhood that isn't often talked about. For the first six months of her life, my daughter suffered from severe reflux. Night and day, the only relief came from holding her upright, which meant sleepless nights and exhaustive days for us both. Having faced depression in the past and HG during my pregnancy, I knew I was at risk of suffering from postnatal depression and I recognised the dangerous path I was on again. The sleep deprivation and isolation took their toll, and I felt myself sinking, struggling silently. It was a daily battle not just against physical fatigue, but against the creeping shadows of postnatal depression. It’s easy to hide behind a smile. It’s harder to speak up. But I’m here to say, it’s okay not to be okay. It's okay to feel overwhelmed and to need help. I learned reaching out, though hard, was essential. The Earth needs our Voice. There is an intimate connection between our throat and our pelvic bowl. When we have stuck energy in the pelvic bowl, it can manifest in the throat chakra as disease or a sore throat, or energetically as being unable to express ourselves. Did you know that the Latin root word “cervix” means “neck” ? 🔥During birth, softening our mouths helps soften the cervix, and birth is easier when a mother can relax and make primal sounds. 🔥When we sing, hum, sigh and laugh, we vibrate our pelvic floor muscles. There is overwhelming evidence that having a doula positively impacts your birth experience. A study in 2017 concluded that those with continuous support in pregnancy and birth were more likely to have a spontaneous vaginal birth. They were less likely to have epidurals and or other pain medications, instrumental births, caesareans, postnatal depression and general, less negative feelings around childbirth. Incredible, right? But, like anything, there are often drawbacks and getting the right support and “fit” is vital. It is the most intimate experience you will ever share with someone who isn’t your partner so you want the best person for the job! I came across a stack of photos during #deathcleaning from my time in Jamaica. It was 1997, reading week in my graduating year from UWO (Women’s Studies), and I was training alongside a handful of international midwives,readying myself for an intended career in Midwifery. I’m learning to suture perineums on a sponge. Later that year, I was headed to El Paso, TX for further training, when I became pregnant with our first son. Plans were put on hold. I was taken under the wing by Elana Johnson, solo midwife here in London from the 1970s until the first graduating class in Ontario, and the past president of the AOM. She was my first doula trainer, and I was in training to be her second attendant. I’d spend another 5 years as a doula, before I’d begin commuting to midwifery school at the now named, TMU in TO. Gosh, this was one of the most beautiful times of my life. I met the coolest people from every activist corner. I ate Thai food for the first time, and learned of love & life in a way I felt I’d missed out on my whole life. I studied so hard, anatomy & physiology almost took me out (still hold onto my study cards). I gave birth at home to my 2nd son at the end of 2nd year. Somewhere between breastfeeding him in a cold car in the middle of the night, 72 hours in to what would be 95+ hours of support for 3 people, I decided midwifery as a profession was not where I was meant to be. My doula career would go on to flourish, as one of only 4 or 5 in the entire region at the time. I was lucky to be at the right place at the right time, in those days. There are so many stories from those early years, successes gained and some back steps. I find myself reflecting more and more, as I guess aging invites, on what has changed and what has remained the same. The fear of birth permeates society, subtly shaping our thoughts and actions, whether we realise it or not. From ancient myths to modern media, childbirth is often portrayed as a perilous journey fraught with pain and danger. These narratives seep into our collective consciousness, colouring our perceptions and expectations. Consciously, we may express excitement and anticipation about pregnancy and childbirth. However, subconsciously, the fear lingers. It manifests in the anxiety we feel when discussing labour, the unease around medical procedures, or the dread of the unknown. Even those who haven't experienced childbirth firsthand absorb these societal messages. Expectant parents grapple with conflicting emotions: the joy of new life and the fear of the birthing process. The fear may influence their choices—whether opting for medical interventions or natural birth. It also affects relationships, as birth partners navigate their roles and responsibilities during pregnancy and labour. The fear of birth is an invisible force, shaping our perceptions, decisions, and interactions. Recognising that fear can play a role in our experiences helps us find ways to improve things. I’m reminding myself everyday that my body is made for this, that I’m a strong woman with a healthy and strong body completely capable to give birth. It sad to think that giving birth has become a mystery when for 1000s of years women have been doing this. It is sad to see how many of us, see birth as something just medical needing a manual on how to. As a doula, I’m here to empower every woman to find their strength, their own power within themselves and to know that their bodies are capable of doing the natural thing on earth. Give birth. And as a doula and a black woman, I’m here to support black women through pregnancy, labour and birth - to have their voices heard, to make them feel seen and to let them know that their feelings are warranted.

Am loving supporting a wonderful family in Angel who as well as having the most adorable baby they also have the cutest fluff ball of a dog who I get my weekly cuddle fix with. When it comes to care during pregnancy, from your first prenatal appointment through birth and postpartum, it is SO important to be your own advocate. We often hear about finding a great provider and a birth team that aligns with our wishes. But today I want you to start thinking of YOURSELF as a key member of your care team. In my role as a birth doula, advocacy is something I see every day. I advocate on my clients behalf AND encourage you to advocate for yourself if something doesn’t feel right. But sadly, I don’t see all care providers advocating for what their patient wants, and I don’t see equal opportunity for advocacy among all women. Women of colour are tragically silenced,which results in dire outcomes far too often. I want to prepare you to take an active role in your care and advocate for yourself every step of the way. Because you know what? Meconium, the earliest newborn stool, passed during labour or in utero can indicate foetal distress and pose risks of meconium aspiration syndrome (MAS) if inhaled by the baby during birth. However we really must separate the simple gastrointestinal maturation vs. more serious conditions to mitigate risks, reduce unnecessary interventions and optimise outcomes for both mother and baby. I have seen all variations of meconium in different settings and different outcomes so I tried to put in carousel a lot of information. 👉Let me know what you think in the comments.
