As a travel nurse, I went on this contract expecting to float to other units if there was a need. In my last staff nurse position I asked about other travel nurses’ experiences floating and I always got mixed responses. Some travel nurses hated floating to new units because they felt that they were poorly prepared to go to those units. Other travel nurses had no problem and enjoyed the experience of learning new things from the units they would float to. Regardless of other nurses’ experiences, I kept an open mind throughout my contract. When I arrived for my contract, I had an orientation and tour with the assistant manager. There I learned that travel nurses would often float, but the benefit of being within the critical care cluster (what they called the 4 ICUs) was that we only floated within the critical care cluster. This meant we wouldn’t have to float to the ED, step-down units, or any other units that we weren’t trained for. Additionally, if we had to float to a more specialized ICU, such as the CVICU, we were usually assigned patients that fit our training and specialty. How many of you are night-shifters? Do you remember how you started your transition to nights? What routines helped you to make the switch from a day schedule to a night schedule a little easier? Personally, I came up with two routines. One strategy that I used is to shift the sleep schedule gradually. For instance, the day before a night shift, I found it helpful to stay up late until the early morning. This can help to adjust the body's clock, making it easier to fall asleep during the day. During this time, it's important to avoid bright light exposure and other stimuli that could disrupt sleep. My second strategy involved shifting my sleep schedule earlier in the day before the first night shift. For this routine, I would go to bed early the day before my scheduled night shift and wake up early the day of my scheduled night shift. I can confidently say that I have experienced imposter syndrome throughout my career as a nurse! I started in the ICU fresh out of nursing school. I barely had the basic foundations of nursing down when I started training and working in the ICU. I learned very quickly that the ICU has a massive learning curve, especially for a new graduate nurse. No matter how much I learned, I still felt like I didn’t know enough. On top of that, having the title of an ICU nurse felt like it came with this expectation that I should know nearly anything and everything! The truth was, I didn’t know everything. When I thought I knew something well enough, I was faced with something new to learn about that very thing. Despite what I learned every day, I still felt like a fraud. I attributed my successes as an ICU nurse to luck and I completely overlooked the progress that I made in my own skills and abilities. I went as far as minimizing my own achievements and downplaying my own accomplishments. Unfortunately, this constant pursuit of self-improvement and growth in order to fit into the role of an ICU nurse quickly led to me burning out. Multidisciplinary rounds, for those that aren’t familiar, is a collaborative approach to caring for patients that involve a team of various healthcare professionals. These would include doctors, nurses, pharmacists, therapists, respiratory therapists (RTs), and social workers to name a few. The primary focus is to improve cross-departmental communication, enhance and progress the patient’s care, and ensure that all team members are working towards achieving the best possible outcomes for the patient. The majority of the patient’s presentation was given by medical residents and interns, followed by the nurse, RTs, and anyone else on the team present for rounds. The attending physician or fellow would usually conclude rounds by discussing overall goals and necessary interventions. As I’ve mentioned before, I was at a level 1, trauma and academic hospital in Portland, Oregon. My last hospital was more akin to a rural hospital, so the routines were slightly different compared to an academic hospital. For starters, prior to starting the shift, we would have a pre-shift huddle. If you aren’t familiar with what a huddle is, huddles are usually brief meetings that are held before the start of a shift. By 7 PM each bedside nurse coming in would pick up an assignment sheet that would list all of the patients admitted, their diagnoses, and pertinent issues to be addressed. The charge nurse would lead the huddle and start by addressing unit-specific announcements, equipment and safety points, staffing issues, and eventually patient assignments. My unit, and all of the other ICUs, promoted independence and nurse autonomy so we were free to advocate for ourselves about which patient assignments we would like to take. I actually enjoyed having pre-shift huddles because we would be able to share awareness of what was going on in the unit. This served to promote unit cohesiveness and teamwork since everyone would be informed about some of the sickest patients and which assignments would be heaviest. I found that whenever I picked up a heavy assignment, I was never without help and I never felt that I was drowning in an assignment! My first travel assignment was at a level 1, trauma and academic hospital in Portland, Oregon. There, I had the opportunity to work with a multitude of healthcare workers. Not that I didn’t have that opportunity at my old hospital, but the difference was that I was able to experience working alongside new medical doctors (MDs). On top of working with MDs and doctors of osteopathic medicine (DOs), I also worked with nurse practitioners (NPs) and physician assistants (PAs). Having spent roughly 6 months working at this hospital, I learned a lot from everyone that I met. Some special mentions and shoutouts to all the respiratory therapists, patient mobility techs, certified nursing assistants, social workers, and so much more! One of the most important lessons that I learned from this assignment is that you have to put yourself out there and get to know the team you are working with in order to learn as much as you can to be a successful and proficient travel nurse! The struggle is real! I’ve lost count on how many times I’ve attempted to lose weight. I’ve lost and gained like a yo-yo dieter, but this time my goal is 2 lbs a week slow and steady. 12 lbs down. Everyday is a struggle. Hopefully, I can keep this up. Now this mirror and camera are making me look smaller than I really am or the pajamas . If there’s one characteristic that I would contribute my success to on my first assignment, it would be my adaptability. Along with adaptability, I would include flexibility. Being able to adapt to this new situation, new routines, and new equipment allowed me to work quickly and efficiently. Flexibility helped as I kept an open mind to how the unit worked, and how they utilized their equipment. My first official week of work started with at-home classes and learning modules for the electronic medical record system and other devices that I would use at the hospital. I have to say that this wasn’t as complicated as I thought it would end up being. Other travel nurses warned me about having to complete modules before work and how tedious it would be, but this particular hospital had very few modules to complete. The few modules that I did have to do were short and straightforward. Over the course of 2 days (roughly 8 hours each day), I finished the modules and classes. Keep reading in my blog! Once we arrived at our rental, I hopped into the shower. I sat there for a good 30 minutes, crying. Negative thoughts flooded my mind. “Did I make a mistake coming here?” “Did I make a mistake bringing my partner into this?” “How do I make sure this doesn’t happen again?” “How do I protect my partner and myself from having this happen?” “This would never have happened back home.” In those 30 minutes, I felt the worst I had ever felt in a long time. Defeated. Vulnerable. Helpless. Until my partner saved me. She sat in the shower with me, still clothed, and cried with me. In my last blog post, I explained how I chose the agency that I went with and what agencies I researched. Let’s dive into the details of my initial contract and the extension of my contract. If you aren’t familiar with how some shifts at a hospital work, a lot of shifts are 12 hours, and 36 hours per week can be considered full-time hours. One offer with a $64 per hour rate was a 48-hour per week contract. Although that is a high hourly rate, the caveat of choosing 48 hours is the lack of freedom to choose to pick up an extra shift which would then end up being overtime. Additionally, the extra 8 hours of a 48-hour contract isn’t considered overtime (at least in the case of this contract, some may vary from state to state). Another reason for choosing my contract was that I would get to work at a level 1 trauma teaching hospital with Magnet designation. This would mean that the majority of the nursing care provided would be the gold standard of nursing care. Additionally, physical and employee resources would be abundant! Lastly, one of the charge nurses that had trained me at my old job was working at this hospital in the PACU. He had the opportunity to cross-train at the medical ICU and only had good things to say about their staffing, resources, and willingness to help new staff. While in nursing school, I always knew I wanted to be an ICU nurse, but before that, probably since spending 6 months in Europe, I've wanted to be a travel nurse. In nursing school, in my third semester, we were mandated to go to a nursing conference In Atlantic City. I remember going by booths, and stopping at a travel nurse booth out of curiosity. I had no idea nurses could do travel, or what it entailed, but I remembered thinking, ”Wow I can travel and ride my horse with the best across the country!”. I pushed the thought to the back of my mind, but it always stayed with me.. I never knew it would be possible, with how much it would take to haul a horse all over, if he would be okay, the details of the jobs, the stress of constantly moving.. but, here I am, in beautiful Northern Maine, riding my most beloved animal with undoubtedly one of AQHA’s best horsewomen.. training with the best, taking my horse on assignments with me, truly loving life every day. I took the leap, one of the best decisions I’ve ever made. I’m here to tell you, don’t let fear hold you back. Things I look forward to now that my contract is done: • Sleeping like a regular human-being • Keeping it 💯. Just not caring for trauma patients. A majority of the trauma patients are there as a result of choices they have made with a “clear mind”. For instance, there’s a whole lot of unhelmeted motorcycle injuries currently. Though the care that I provide doesn’t change based on why someone was admitted to the hospital, it’s a lot harder to have the same empathy for someone in a different situation where they were attempting to be safe, running from a bad situation, and/or did not have a “clear mind” (fell off a ladder, attempted self-unaliving, ATV rollover etc.) • Not doing q1h assessments (neuro checks, lumbar and EVD drain draining, and ICP checks/CPP calculations). I don’t think I’ve ever had to do so many frequent assessments in my nursing career. 😂 • Not having to float mid-shift to another unit • Reading more. Recommendations are appreciated BTW. ✨ • Practicing Spanish regularly • Participating in physical activities. I could not find a good routine with exercising. I can’t wait to continue my OTF classes and start swimming lessons! • Potentially looking for my own place. 👀 • Spending some much needed time with friends and family. ❤️

Reflecting this morning. My good friend reached out to say he had already called time of death on two patients by 12:30 this morning. The reality is, it still hurts even if you’ve seen death more times than you’d like to admit. Pouring every piece of mental & physical energy you have into saving another human and it not being enough. At times, I feel like we are constantly silently grieving. The career you love and invest so much heart into, that can bruise your soul in a matter of minutes. It’s the beauty of it. The adrenaline. The unknown. The heart. Passion. Teamwork. Medicine. The commitment to keep showing up for your patients. We grieve. We share amongst each other only, because we know that’s the only safe place. We love harder. We see life & all it’s precious gifts. Sunrises and human connections are that much sweeter. We cope, in healthy and unhealthy ways. We’ve become kings and queens of compartmentalization. We overcome. We show up. Fight for you and your loved ones like they are our own. We are still human. To everyone who’s stayed at bedside during this pandemic, we are 2 years in, life has returned to “normal” for the rest of the world, but we hold moments and memories like tattoos in our brains. To those that left to honor yourself. Kudos. It’s hard to leave what you once loved. We’ve lost a lot, we’ve maybe even lost pieces of ourselves, but what we’ve gained is connection - perspective - knowledge. We have become masters at adapting. We’ve created communities on platforms to fight for our rights and those of our patients. To all of my patients I worked tirelessly on and still lost, I still remember you. Holding your hand, FaceTiming your kids and spouses, attending the prayer services/vigils with your loved ones, crying with you.🤍 Today and every day..
