This made me chuckle! I see a lot of people with creaky, grinding knees - especially on movements like squats. One question they always ask is "is it ok that they creak? Is it safe for me to squat?" Simple answer is yes...provided there is no pain. It might feel funny, even unpleasant - in a 'turns your stomach' kind of way. But for the majority of people who experience these sounds and sensations, exercises like squats are perfectly safe to perform. It is even recommended as building strength in the muscles surrounding the knees will help prevent injury and support and existing cartilage degeneration. Our spines are amazing structures...so strong and flexible too! But much like any other joint, if we don't move the spine throughout its full range regularly, it looses some of this flexibility...literally use it or lose it! Unfortunately when back pain strikes, it is very easy to become wary of moving your spine in the way you used to. We often avoid certain positions or activities for fear that we may cause further damage. Sadly, that often leads to further disability or a reduced recovery as the back is less mobile and weaker than it was previously. Where really, we ought to be increasing our strength and flexibility. Of course, after a significant episode of back pain it is normal to be concerned. Our Top 3 Tips for Sports Injury Prevention! The first thing to note is that of course, not all injuries can be prevented. These are the ones which occur from a moment in time - a tackle for example. However, many other injuries can be avoided by following some simple guidelines: 1) Gradually Increase Training Volume and Intensity Whether you are a beginner or you are experienced in your sport, always increase your training volume and intensity gradually. Big increases overload your soft tissues and bones, making injuries like tendinopathies and stress fractures more likely 2) Include Strength Training in your Program It’s always a good idea to include 1 to 2 strength training sessions a week into your program. whether you’re running or playing a team sport - this helps to strengthen the muscles and tendons you use during your sport. Just doing the sport along isn’t enough! 3) Get Enough Rest Rest and recovery are a key part of injury prevention. If you are overworked and fatigued, you are more likely to injure yourself. Rest time is also when strength gains occur. 👣 Plantar Fasciitis 👣 Most people will have heard of this condition - and if you, or anyone you know has suffered with it - you will know how horrible it is! Plantar Fasciitis (PF) is often mistaken for other conditions - possibly because it is so well known, an other injuries are less heard of. PF can be treated, albeit sometimes it is very stubborn! But to have a chance of treating it effectively - we need to make sure that it is PF in the first place! The most common symptoms are listed on the image. The pain is always under the heel - not on the back. Pain at the back is more likely from the achilles, or medial or lateral tendons. PF comes on gradually - usually due to an increase in load - it could be a new job resulting in more time on your feet, or a change in shoes for example. It is always painful in the mornings - those first few steps when you get out of bed are agonising. It then slowly eases off the more you walk on it (up to a point!). You may find it is then worse again if you've been off your feet for a while. If these don't sound like your symptoms - it might not be PF! 🎾 Tennis Elbow 🎾 Probably the most poorly named condition I see! Tennis elbow is rarely ever caused by tennis! It's actually caused by repetitive wrist movements - something which shouldn't really happen that much in tennis. However it can sometimes occur - most often when using a racket with a grip which is the wrong size, or playing in the wet when the ball becomes heavier. I most commonly see this condition in people who have had a sudden increase in wrist motion - maybe they've done some decorating or DIY at home - lots of painting or screwdriving! The pain is found on the outside of the elbow, and is usually most tender about 1-2" below the bony lump on the outer elbow - known as the lateral epicondyle. You'll have pain on extending your wrist (pulling it backwards), gripping and lifting items like a heavy kettle. The good news is that it can be treated and in 9/10 cases (at least) - conservative treatment such as activity modification, strength exercises and manual therapy do the trick! Hands up who does this? 🙌 You're probably in the minority if you say no! For me - It's front squats and split squats mainly🤮 Problem is...we find specific exercises harder than others for a reasons. And that reason is often because we are weak into that movement pattern. And what's the only way to get stronger in your squats (or whatever exercise it may be for you!)....to train them! Remember...a 'weak' or underperforming muscle (/muscle group) can be a contributor to a pain condition or injury... Break or Fracture? Both! A broken bone is the same as a fractured bone - the terms are interchangeable. I think sometimes people think that if a bone is 'only fractured' then it isn't as bad as a break. But in reality "fractured" is just the medical term for broken! Definitely some forms of fracture are worse than others... stress fractures, avulsion fractures and closed fractures are usually relatively quick to heal with few complications. More complex fractures such as open (compound - where the skin is broken with the bone coming through) and comminuted (where there are three of more fragments of bone), can require surgery and metal fixation - and as a result longer recovery times. Do Your Hip Flexors Feel Tight? This is another common complaint I hear in clinic - hip flexors feel tight, you're constantly stretching them, with no change... Things I've Learned in my 18 years as an Injury Therapist! Where do I start? When I look back at when I first started out after I graduated, I cringe! Straight out of uni, yeah I had a bit of knowledge but not a great deal of skill when it came to applying it. Plus...things have changed, as a profession we've learned and grown and I certainly don't perform IT band stripping anymore 🙄 😆 1) I can only do so much...I can perform the best manual therapy treatments, show the most suitable exercises, advise on lifestyle and reassure....but I can only do that much. The rest comes down to the patient. 2) Sometimes less in more...I learned this the slow way! There's no point giving someone 20 exercises to do - they won't do them! Give them 2...and they stand a greater chance! 3) When to refer on...Sometimes, my skill set is not what someone needs. Thankfully I know a whole host of other local practitioners who can help! 4) Communication is key...This is one of the hardest skills to master. How to communicate with people so that they trust you, they respect you, they are clear on what you are telling them and they walk out feeling more informed and better understanding their issue and the way forward. 5) Exercise rehab trumps manual treatment...Every time! Everyone loves a massage and rubbing something when it hurts almost always leads to some temporary pain relief. But it's not always the best way forward. Lower back pain Myths vs Facts! There is so much false information out there surrounding lower back pain - outdated ideas, unproven opinions and some frankly dangerous / damaging advice. Here are just a few which I hear / see from time to time... Most notable points here are that pain exists for a lot of reasons and it isn't always directly linked to damage. You can have excruciating pain without any physical damage at all! There are many other factors to pain (psychological, social etc) and we still don't fully understand it. Secondly, the spine loves to move...it was designed to do just that. The only movement that is dangerous is one that you are not used to / have not trained for - for example repeatedly lifting heavy boxes on moving day. Unless you've been training your deadlifts - you've probably not prepared for that! Posture - there is no bad posture. There is no proof linking any one particular type of posture with a higher risk of back pain - EXCEPT for static postures. I'll say it again - the spine likes to move! And finally - Joints and other body parts DO NOT move out of place / alignment. How unstable would we all be if things just randomly slipped out of place all the times! What is an MSK Physio? An MSK Physio is a Physiotherapist who has a degree (usually a three year undergraduate, sometimes a 2 year masters) in Physiotherapy. They are a member of the Chartered Society of Physiotherapists (CSP) which regulates the profession, registers all Physios, ensures every one of their members has professional insurance and provides a complaints procedure for members of the public. They specialise in treating and rehabilitating injuries to bones, and soft tissues - such as muscles, tendons and ligaments. MSK stands for Musculo-skeletal. A good Physio is a great listener. In most cases they have a good idea of what is going on by just talking to you, listening to your symptoms and asking the right questions about your issue. They are also empathetic to your concerns. Whilst they may not have been through the same pain or injury themselves - they will very likely have treated many people who have and so have an understanding of the emotional side of pain and injury. Q: Do we NEED a diagnosis when treating lower back pain? A: In most cases - No! Back pain is one of the most common pain issues and biggest reasons for people to be off work long-term. It can be incredibly debilitating and scary. There are many different reasons why someone may suffer from lower back pain - some serious, most less serious. For most people, an episode of acute lower back pain will last anywhere from a few days, to 3 months and in most cases, it will resolve on it's own accord. With so many structures in the lower back which could be contributing to the pain, it is very difficult (almost impossible) in most cases to ascertain EXACTLY what is causing the pain. Even if a patient has an MRI scan which reveals a pathology such as a disc herniation, this cannot be guaranteed to be the source of the pain. MRI studies have shown people with zero lower back pain can still have a herniated disc! The treatment for most cases of lower back pain (once red flag symptoms are cleared), is much the same... 🌟Keep moving - however is comfortable 🌟Use painkillers where needed in the short-term 🌟Follow a progressive rehabilitation programme 🌟Work within your pain threshold So no, for most cases we don't REALLY need to know EXACTLY what is going on, as with some active rest and time the pain will usually subside. STOP stretching your calf to try to combat Achilles Pain! Achilles tendon pain and stiffness is a common problem, which is often worse after periods of rest and first thing in the mornings. When you get up, it feels like you need to stretch it out, right? But STOP! Stretching your calf / achilles complex is NOT going to help your condition in the long run and could actually prevent it from getting better! To be efficient, the achilles needs to be stiff and resilient. We don't want a loose, floppy achilles! Stretching the calf and tendon is therefore not helpful. Even if you do have "tight calves" - this is probably NOT the cause of your tendon pain. This is more likely due to a sudden increase in use (due to change in habits / exercise, footwear), and / or a lack of resilience / stiffness in the tendon. Achilles Tendon Injuries... The achilles tendon is *probably* the most injured tendon in the body. That's no surprise given the load it withstands on a daily basis. In fact, the achilles tendon deals with loads up to 10 times our bodyweight when performing exercise such as running and jumping. In clinic I regularly see cases of Achilles tendinopathies which develop due to excessive load on the tendon. Often this is seen in runners and other sports players but can also occur in those who do not participate in physical activities. Tendinopathies are especially prevalent for those in the 50 plus age range. Achilles tendon tears and ruptures are less commonly seen in my clinic but must be thoroughly rehabbed in order to make a full recovery either post-surgery or post-immobilisation. Due to their prevalence, I am now undertaking a CPD (continuing professional development) course to refresh my understanding of achilles tendon injuries and their rehabilitation. The WORST thing you can do for most injuries is REST! This may seem like a controversial statement but let me explain! Let's take an example of achilles pain...probably a tendinopathy of some sort which is an 'overuse' injury. What this means is that the tendon has come under more strain / load than it can tolerate and it starts to hurt....At this point, most people will decide to 'rest' it which allows it to settle down and at some point in the future (be it 2 weeks, 2 months or 2 years!) they try to return to the activity which caused the pain. After some length of time, the pain usually returns. The reason for this being that nothing has changed! The tendon couldn't cope with the demand being placed on it, so rather than resting it and allowing its load tolerance to decrease even further, we should be developing its strength and therefore how much load it CAN tolerate! A tendon causing a lot of pain may need a few days or a week or so to settle, but strengthening can begin at a low level, within the tendon's pain tolerance. This is gradually increased over time. The pain causing activity (e.g. running) can often be moderated to allow some level of continuance also (maybe walk/run), meaning you don't lose the fitness / strength you already had.
