Može se pomicati negdje do 32. tjedna trudnoće, najčešće prema gornjem dijelu maternice, dalje od cerviksa, kako beba raste. • Neke od komplikacija posteljice su: • Placenta previa (predležeća posteljica) – posteljica potpuno ili djelomično prekrije cerviks (slajd 3.) • Placenta accreta – posteljica se „zakači“ preduboko u stijenku maternice (slajd 4.) • Placenta abruption (abrupcija posteljice) – prijevremeno odljuštenje posteljice od stijenke maternice (slajd 5.) • Placenta insufficiency (insuficijencija ili loša funkcija posteljice) – posteljica ne pruža dovoljno nutrijenata ili kisika bebi • Retained placenta (zaostala posteljica) – zaostali dio posteljice u maternici nakon poroda iste (slajd 6.) • Najčešći znakovi da nešto nije u redu s posteljicom su uglavnom znakovi krvarenja. E sad, to ne znači da svako krvarenje i u bilo kojoj količini ukazuje na problem s posteljicom. Svakako, preporuka je da ako ti se dogodi da vidiš krv na svojim gaćicama, a trudna si – odi i provjeri. Researchers at @pennstate found that clinicians who were prompted by electronic nudges from a machine learning algorithm that predicts mortality risk quadrupled their rates of end-of-life care conversations. The study also found that the reminders triggered by machine learning significantly decreased use of aggressive chemotherapy and other interventions that could lead to hospitalizations in patients’ final days.

New podcast episode! This week, I sit down with Tien Nguyen of The University of Texas Austin to talk about how Covid 19 affected access to doula care and it’s impact on maternal health. I was so grateful that Tien took the time to share the findings from her study with me for The Rosewood Doula Podcast so I can share it all with you. We get into some of the nitty gritty around how birthworkers have had to adapt to hospital restrictions through the pandemic, how communication strategies improved or got interrupted over the course of the past year, what takeaways doulas have around the switch to virtual care, what policy makers can pay attention to around birth support following what we’ve discovered amidst the pandemic, and a lot more. What’s super exciting to me, too, is that Tien is hoping to become an obstetrician and researcher when she’s done this portion of her studies! And the impressive stats just keep coming 🌟 It’s amazing that there has been such a comprehensive study done to assess the value of continuous support through labour and the effect of the individuals delivering this support on birth experiences and outcomes. Cochrane Library’s (2017) systematic review of this subject found doula support increased women and birthing peoples experience of birth by 35% 🌟 With the average increase in this rate for any type of support during labour at 31%, it’s a pretty loud and clear statement that women need continuous support in labour, whether that’s from a partner, midwife, friend, or charity hired doula. There are some amazing doula charities set up in major cities, which include Neighbourhood Doulas in London and Amma Birth Companions (set up by my mentor Helen MacDonald❤️) in Glasgow that serve vulnerable women and refugees. Some of you might be wondering how and why having a doula at your birth can make this much of a difference. And this is something I’m going to cover a bit more in upcoming posts, but here are some highlights below: To start, doulas only offer emotional support, advocacy, movement options, environment setting and information support. A doula is there for you and only you. We can support any type of birth, but are extremely knowledgable when it comes to the physiology and psychology of natural birth. We are also trained in birth rights and will fight your corner. And it turns out this makes a heck of a lot of difference for birthing women. Often people overlook how enormous the task of the birth partner is, they often need support to fully show up for you, the birthing mother. And midwives are actual superheros, but they can’t always be with you and will have other duties and policies to adhere to. It might sound like an obvious one, but support is a game changer when it comes to birth. Positive. Unwavering. Uplifting. You’ll need your cheerleeder(s). Of course if you want to be left alone, that’s perfectly fine, and actually pretty normal. Support can be a presence too, just knowing someone is there if you need them. It can be the right touch, movement suggestions, information and advocacy. In the Cochrane Systematic Review of Continuous Support Through Labour (2017), the reviewers found that continuous support increased the chances of a vaginal birth by 8% (15% with a doula present). The risk of Cesarean decreased 25% (39% with a doula). The use of sensation relief reduced 10%. And Labour was shortened by 41 minutes. Additionally there was a comparative 38% drop in the low 5-min Apgar scores (a means of testing how healthy a baby is) 🌟and a 31% less chance of being dissatisfied with birth. It was a whopper of a read, 120 pages, and full of technical and medical research terminology, but well worth the time! Saw @drsarawickham posting about this recently published article and I just had to share the love. Evidenced based research demonstrating home births after cesarean (HBAC) are associated with higher rates of vaginal birth 🌟 The Maty in Orkney have increased their VBAC rates over the last year. Very interesting that the 4th goal women stated was they want to feel a sense of control, which is related to the #3 goal of feeling prepared and being involved. This desire is strong and stated in different ways. Women do not want to feel like procedures are being done to them. That quote from Dr. John Kennell has been around for many years, and in the time since he's said it dozens more articles have been released clearly showing that ⁣ DOULAS⁣ MAKE⁣ A⁣ DIFFERENCE⁣ IN ⁣ OUTCOMES⁣ IN⁣ THE⁣ RESEARCH.⁣ ⁣ And the research has shown *no adverse outcomes*. ⁣ ⁣ So let's reflect on that quote - if there was some kind of medication or intervention that reduced cesareans by 39% (!!!!!!!!!!), it would be STANDARD CARE. Sheila touched on many countries, many time periods, and many beliefs and traditions around birth. She was a wealth of knowledge, had spent much of her life educating and advocating for change. Change away from the lithotomy position, forceps use, twilight birth, medications that caused harm to mother and baby. All these things that became part of birth during the 1800s which caused more harm than good. So it’s an interesting read. We can learn a lot from those that came before us. The experiences they had. A study a few years ago stated that 27% of moms who did not use a Doula, wished they did! We’re willing to bet it’s because people assume they can’t afford it. Doulas are actually very affordable when you consider the benefits you get!
