Sweet Olive had a really 'wonky' toe (Yes, that is a technical veterinary term) that continued to get worse over time. I got to do a digit amp in a little cat today 🙌 As you can see this little guy had a mass on his toe, so needed the toe removing. I made a y incision around the digit, blunt dissected, cut through tendons/ligaments, disarticulated the joint and then closed subcut layer and skin. Following on with my lame cow mini- series 🐄 This is what the amputated claw looks like. I often bring the claw back to the clinic to take x-rays of it. I can then show the owner how severe the pathology was and I also get to see exactly where I made the cut on my amputation. I wasn't happy with leaving a small bit of P2 in this case(it was apparent at the time of surgery). This happens sometimes as you are at the mercy of the position of the foot relative to the bars of the crush!Solution was to disarticulate the fragment and then excoriate the cartilage of the joint surface of P1 to speed up fibrosis and healing. Some people knock down cows to have more control of the angle of the cut. More on that next time! Image 2 shows the claw with a haemostat placed in to it to show how far in the tract goes. Image 3 shows the xray with infection obvious tracking into the joint between P2 and P3. Image 4 shows the haemostat in place so you can see clearly the external wound was going directly into the joint. Nasty septic distal interphalangeal joint with heel bulb abscess, and septic tenosynovitis of deep digital flexor tendon. It isn’t clear what the exact cause was but this cow has a notable lateral hoof wall with granulation tissue present indicating chronicity to this lesion. There is also notable swelling of the leg that is mostly over the lateral claw which is pretty consistent with a septic joint. Often the septic DIPJ animals also get hair loss just above the coronary band as we can see here a little.The cow has been treated several times before this but failed to respond to treatment. We always like to take a radiograph or x-ray to confirm our suspicion and make sure there are no obvious signs of osteomyelitis or sepsis in the medial claw/joints (make sure the cow has a good spare tire!). You can see in the radiograph that there is obvious widening of the joint space and even a gas pocket present confirming the suspicion of a heel bulb abscess. In. This case even though the proximal interphalangeal joint appears normal it is elected to do a high digit amputation through the distal end of the first phalanx. Although the procedure is not captured here we do a clip and surgical prep, regional limb perfusion, an additional prep and then a scalpel is used to make a track through the skin and tissues so an OB wire can be used to cut through the distal end of P1. Once amputated a small section of the end of the DDFT is also removed as it appeared to be necrotic. Otherwise the remaining tissue is healthy and a toe gut bandage is placed on the leg with gauze, cling, vetrap, and duct tape. Of course a round of long acti by pain medication and antibiotics are used on these animals. The bandage is placed on right and with lots of gauze as this is the only means of hemostasis and we just makes a big open wound! We usually leave bandages on for 3-4 days, replace the bandage for an additional 7 days then remove it and leave the bandage off. Typically animals by this time are getting along quite well and in a few weeks most can’t be notice to be lame anymore. #digitamputation #cowvet #cownerd #vetlife #vetschool #vetsofinstagram #cows #surgery
💪🏼 Team work makes the dream work 💪🏼 On Tuesday, Richard and Emily went out to a dairy cow with a chronic foot injury that after initial examination needed a digital claw amputation 🐄 This procedure is carried out with local anaesthesia via the use of a tourniquet and local intravenous anaesthetics 💉 Once removed and bandaged, Richard remodelled the remaining toe as well as giving pain relief and antibiotic cover 🩺 The cow left for the field a lot more comfortable and weight bearing on the remaining claw 👌🏼🙌🏼

Don't swipe if you are squeamish. A classic call this time of year. You get maybe 1-2 of these ever Winter. Animal caught it's toe in the slats and twisted it almost amputating it itself. In the video you will see how it nearly removed the horn of the hoof completely. In these cases best treatment is amputation. A few points of interest: 1. Use a butterfly catheter pointing towards the toe to do your IVRA. It will blow your mind if you have been using needles to date! 2. I like to radiograph claws post op to see if I hit my target. I aim for distal P1. I pretty much wired right between P1 and P2 today so I was about 5mm too low. It will still heal fine but P1 is my preference. This was a 300kg bullock and I find I often hit P2 on these as it seems scary to go so high on their smaller feet! I cut between the toes to seat the wire deeply but just missed the angle today. It is more important to cut higher if amputating for deep digital sepsis. 3. I sedated this bullock so he was recumbent for the procedure. Sometimes it's easier have full control of them. 4. Obviously post op he can never go back on slatted flooring! Digit amputation 💉🔪 This cow presented with a chronic solar abcess and white line disease. Despite appropriate treatment by the farmer (shoeing and medication), things did not improve. In the interest of the animals welfare amputation was performed! I clipped the hair to just above the fetlock to provide better visualisation of the superficial veins and to help clean the area. I place cable ties just below the carpus to allow for IVRA to be preformed. I then placed a 21G needle into the vein and allowed approx 12ml of blood to drain before injecting 12ml of adrenacaine local anaesthetic into the vein. Working fast to clip and clean away the area to be cut I made an incision with a scalpel between the claws and up the front and back to 2 fingers width above the coronary band and connecting either point across the surface of the damaged claw down to the bone at the level of distal -P1. A length of iodine soaked embryotomy wire was passed into the wound and used to amputate the bone. After removing the appendage I cut away some of the fat present as it tends to aid post op infection. As we managed to cut well above the infectious margins I placed some absorbable sutures on the excess viable skin to speed the healing and get her back to comfort ASAP. I placed a wet to dry primary bandage, cotton wool and tightened Elastoplast over the wound before removing the tourniquet. Fortunately we had a paring crate as medial front claws tend to be the most difficult to access for removal. I normally would also use sedation in wilder animals but this lady was an excellent patient. She has to get nsaid and antibiotics for 5days and bandage replacement at the end of her course to remove corruption and reassess. Digit amputation is always a salvage procedure. A front leg amputation tends to have a poorer prognosis than a hindlimb due to the distribution of weight being carried. In general an animal with a front claw amputation will get 1-1.5 lactations before and a hind claw amputation tend to get 2 lactations! #digitamputation #clawamoutation #largeanimalvet #farmanimalvet #veterinarymedicine

The importance of being thorough. This dog was referred for a TPLO for its cranial cruciate ligament disease. An incidental finding of a swelling on its fore paw which was aspirated at the referring clinic was suspected to be a grass seed foreign body reaction based on the finding of neutrophils. When I examined the dog, I was concerned by the appearance of the digital mass and biopsied it instead of doing a TPLO. Unfortunately this digital mass was a histiocytic sarcoma and the dog had the digit amputated. The dog is now undergoing chemotherapy for histiocytic sarcoma, and we are managing his cruciate disease conservatively.
I'm probably in the minority of vets in that I like treating lame cows. It's messy and slow work but it's rewarding getting results. I would love to work in a UK-style system where the vets and hooftrimmers work together and so can share knowledge, equipment, refer cases to each other and have all the clinical records in one spot. . . This cow had been treated with antibiotics, no response, on exam it quickly became apparently she had a septic joint. When I removed some heel I could find a bleb of pus. Putting pressure on the heel bulb with one fist and flexing the toe with the other caused pus to burst out dramatically. This is the best way to check if infection is communicating with the joint. I don't think even an arthrodesis would salvage that cows foot so digit amputation quickly relieved the pain of the septic joint. Amputation was performed through the distal aspect of P1. Who likes hoofwork and who avoids it at all costs? This patient sustained a work related amputation. She was having issues with pain at the end of her digits. The prosthetist custom fabricated these prosthetics. This handsome 9 year old Dachshund presented to me last week with a swollen and painful toe. I took radiographs and discovered massive bony destruction of her distal phalanx on her third digit --- likely caused by bone cancer. Today we amputated the distal and middle phalanx and will send the toe to the pathologist so we know what type of cancer we are dealing with. Warning: graphic images!! An area of veterinary medicine and surgery often overlooked by the profession is bovine lameness. I definitely feel vets have a bigger role to play here. Ideally we would together with a trained professional hoof trimmer as a team where they do the routine pares and preventative work and refer any surgical treatments to the vets on the team. That way everyone is on the same page and their time is being best used. These are images from a farm with ongoing issues with toe necrosis(apicalis necrotica). Likely secondary to digital dermatitis. Toe necrosis cannot be managed conservatively. In this case I previously performed four partial toe amputations as salvage procedures to try avoid amputating the full digits. Two out of four responded well which is a good success saving two cows from full amputation. Yesterday myself and @mikeslancaster went out to carry out further treatment. Image 1 shows one of these cows having regrown a functional digit which looks pretty good and no further necrosis. Image 2 and 3 show the radiographs and the amputated digits from the two cows which had previous partial digit amputations which didn't respond. Image 4 shows a new case, earlier stage of toe necrosis but it was decided to go straight for digit amputation. Digit amputation is a really successful treatment option for cows which I strongly recommend but sometimes we try advanced surgical interventions to try save the digit first as being a toe short is tricky in Irish herds wintered on slatted floors. There are many options for residual digit splinting following amputation. Indications for splinting include protection, scar management, shaping, phantom pain and/or nerve pain, and of course enhancing functional grip and pinch within the context of the patients daily occupations. We have tried custom DIP protective splints, compression digit sleeves, silicone digicaps, and Coban wrap. Guess what this patient likes best? Major sass this morning! She was not happy about her follow up appointment today but she was excited to see her favorite vet. All looks good with her paw and we’re planning on continuing the cytopoint injections on a monthly basis. I swear we have the BEST vet ever. Mari calms down as soon as she sees him and lets him do what he needs to. Also, a 30min appointment, medicated shampoo and an ear cleaning today at no cost. Hey guys! Lola here! 👋🏼😽😻 So I have come on leaps and bounds since my last update! I had my toe amputated which was causing me many problems over the last couple of years. This weekend however, my pawrents said it would be fine for me to go outside as my stitches were removed last week and the vet was super happy with how my toe had healed! Radiograph of a yearling Bullock which caught it's toe in the slats and broke the hoof wall almost clean off. Intravenous regional anaesthesia was given and the digit amputated through distal P1. As you can see on the radiograph he had not only broken the external hoof but broken P3 aswell. The time came where the swelling in Rooster's foot become very bothersome very quickly. After chatting with his vet and consulting with an orthopedic vet, we decided to remove the digits on the bones of the affected foot. This means P1, P2 and P3 on both bones needed to be amputated in hopes that this would relieve the pressure of the contracted bones, allowing them to try to realign. Rooster had an amazing doctor at @westparkvet (oh & an amazing vet tech 😉)!
