But as temperatures soar, so does the need for a critical resource: water. That's why we're turning to our community for help. This Saturday, June 1st, from 9am-12pm, we're hosting a Thirst Aid Public Water Drop-off event at the Keys to Change Campus (1300 W Harrison St). By donating water at this event, you're directly supporting the work captured in this photo – you're helping us ensure that our Street Medicine teams can continue to provide not just medical care, but also the hydration that's so vital for survival for those experiencing homelessness. Ready to support those in need and arms full of supplies, we set out to make a difference in the lives of the unhoused individuals in our community. We distributed water, snacks, and medical supplies, and medical care. We provided social care, offering support to address the multifaceted challenges faced by our neighbors. We MacGyver-ed the hell out of some cardboard and duct tape to make a makeshift seat during outreach yesterday. But in the end, we just stole a chair. Situation: After two hours of outreach, we decided to make one last stop in the International District before heading to Coors for another two hours. We saw a woman hunched over a walker, her head almost touching the cement. We’ve seen disabled elderly women like this many times before. They are often plagued with chronic back problems but have a support system on the street that watches over them. It was the volunteer handing her candy who discovered the woman was only 24. She needed the walker because her left ankle was shattered after she was hit by a car last week. That leg also had visible medical staples and her right thigh had a large scar. Her lower left leg was swollen with fluid, making the skin shiny and tight. She needed to elevate it, but sitting on the curb was impossible due to her injuries, and her walker didn’t have a seat. We went into DIY mode, searching for something to create a seat. We searched the bushes and tore apart the outreach car, but only found a box. So, we duct-taped the box to death and hoped it would hold her weight when we set it across the bars of her walker. It didn’t. While we contemplated dismantling a nearby “no trespassing” sign, someone jokingly suggested stealing a metal folding chair they spotted outside an apartment door. The woman mentioned it belonged to a friend who had died of an overdose while she was in the hospital. We audibly said “We’re sorry” and “Thank you” the whole time we did it, but we stole the chair. We helped the woman sit down and elevated her leg on her backpack. Then we called ACS for a wheelchair. We also reached out to all our contacts. But it was 5 p.m. on a holiday weekend, so we all struck out. As soon as we find a wheelchair, we will go find her again. Which won’t be hard. This was adapted from a list developed by @nycactionmedical Start with a backpack that has organizational structure and lots of pockets. You might want to consider wearing a fishing vest. In the outer pockets/loops store: a marker, pencil, small notebook, trauma shears or lockable knife (police may consider this a weapon), and a water bottle for drinking. Inside your bag: a 1 litre water bottle for washing wounds/hands, and another 1 litre water bottle with a squirt top for flushes, and 20 pairs of nitrile gloves that fit you (also keep pairs in your pocket in case you lose your pack!) Pack kits using ziploc bags: fill gallon bags with smaller bags packed with enough supplies to treat one injury. For general health: cough drops, slippery elm lozenges or slippery elm bark, lightweight snacks, honey packets, small candies and ginger for nausea, menstrual pads, N95 masks and small cups for giving water or drink mix. For minor wounds: gauze squares or cotton balls, bandaids, alcohol wipes, antibiotic ointment packets/tube, adhesive strips (butterfly closures, steri-strips, medi-strips). 1/2” or 1” micropore, transpore or silk tape and balm (like Arnica). For major wounds: roll of 3” medical tape, 20 safety pins, ACE wrap, instant cold packs, 5 triangle bandages/cravats. 3 bags of each: 1 sterile 4x4” gauze, 5 nonsterile 4x4” gauze, roller gauze, 1 occlusive (air/water tight) dressing. For chemical weapons: goggles with an airtight seal, water bottle with squirt top for flushes, or a sealed water bottle, gloves, bandanas (to soak), garbage bags (for contaminated clothes/equipment), sudecon if available (DIY recipes will expire in a few days unless refrigerated or frozen.) Also good to have: 20-40 earplugs (for noise demos/LRADs), small bottle of liquid soap in a ziploc, powdered electrolyte drink mix, hand sanitizer, instant hand warmers, spare t-shirt, naloxone, SAM splint, thermometer, sunscreen, pen light or flashlight, CPR shield/one way valve mask, phone power bank and charger, clotting factor #streetmedicine

On Friday, we saw our first case of skin ulcers likely caused by xylazine. We have a couple of reasons to think this. First, the woman told us they started off as small “burns,” but she doesn’t remember getting burned. Second, the ulcers are growing quickly — initially they were about the size of a dime, but now, three days later, are the size of quarters. They’re also getting deeper. In the beginning, they just involved the top skin layers, but the tissue is becoming more concave. The best way to treat these ulcers early on is to keep them moist. This means putting a thin layer of Vaseline or something similar on it, then covering it with a bandage. Keeping the ulcers moist stops the skin from drying out, which can make the wounds get bigger and deeper. In the woman’s case, this would be really bad because it’s on her hand. If it gets bigger, it could eat away the tendons and ligaments around her fingers, possibly causing permanent damage to her hand. Thankfully, she took it very seriously when we told her to get it checked by a doctor, and was planning a trip to a clinic. #xylazine #tranq #streetmedicine

We came, We saw, We conferenced! 🕺 Our first time officially attending a conference and wow what a wonderful, inspiring and at times frustrating experience. People from all over the country convening to put our heads together and share various ways communities nationwide tackle the beast that is homelessness & our beyond shattered healthcare system. So many people doing really amazing work despite all odds! Long and hard discussions about the impact of sweeps, the need for housing and the ways in which criminalizing folks on the street doesn't work. Representatives from small orgs, large healthcare systems, policy makers, lawyers, public health officials at county, city, state and federal levels were there.. . You know who wasn't there? Representatives from our own @cityofeug and @lanecounty (at least as far as we know, but we would LOVE to be corrected) They could have learned so much. This is why we love the ACS Street Outreach and the UNM Street Medicine teams! Yesterday, we stopped to give supplies to a woman we first met several months ago. Her left leg is double its normal size from lymphedema. The skin on it is stretched so thin, it breaks open on its own without being cut or punctured. She does her best to keep it clean, but she stays in an abandoned house — called an abando on the street — where there is no running water. She also doesn’t often have clean bandages and gauze since her mobility problems mean she can’t go get them. All of this has led to a lot of hospital visits. The last two times were for maggot infestations. That’s where the ACS Street Outreach team comes in. With the women’s permission, we gave them her usual hangout spots, and they then alerted the UNM Street Medicine team. Their combined Wonder-Twin powers mean not only will the woman get her leg clean and bandaged, but they will also come up with a plan to keep her healthy and see about getting her housed. This, in turn, will save you — the taxpayer — a lot of money. Because all those hospital visits — at least two a month — add up to about $79,000 a year in Medicaid costs. (Fellow math geeks, see below) This is why we love the ACS Street Outreach and the UNM Street Medicine teams. For all those reasons … and because it means less wracking guilt for us. We were able to leave the woman — after giving her the usual supplies, plus bandages, gauze, and lots of water to clean her wounds — knowing we weren’t the only ones looking out for her. Without the two teams, we would stay up at night worrying. And wondering if we’d done enough. Pottstown’s Street Medicine and Street Psychiatry program received the 2024 Mental Health Innovation Award from Montgomery County. The award was given to Street Medicine for “Trailblazing a new standard of care in Montgomery County: Prioritizing health equity enhanced outcomes and innovative service delivery." As a CalOptima Health Director and a County Supervisor for Anaheim, I attended the kickoff meeting for CalOptima Health Street Medicine Program for the City of Anaheim. The CalOptima Street Medicine Program delivers comprehensive healthcare to individuals experiencing homelessness through a street medicine model. Services include primary care, behavioral health, case management, and expanded whole-person care resources now available through CalAIM, a state initiative offering a new set of Medi-Cal benefits that includes housing services. The program first began in Garden Grove in April 2023 and was approved by the CalOptima Health Board of Directors to expand to Anaheim and Costa Mesa in December 2023. The Street Medicine program is one of many programs that Orange County is implementing that is helping us address homelessness and expand accessible healthcare. #TBT to our HICHC leadership and staff attending the Kona-Kohala Chamber of Commerce's Luncheon with the Mayor. We were joined by law enforcement, business owners, and other community partners to discuss homelessness on the Big Island. Our CEO, Richard Taaffe, spoke on one of the panels related to finding solutions to take care of our houseless community.⁠ ⁠ Through our Street Medicine program, we can reach our houseless friends where they are; whether it's on the streets, at the beach, in encampments, or at a shelter. As many are now aware, today was a tragic day in our hometown of Charlotte. We are choosing not to be a source of information or “break” any new details surrounding this matter. We are choosing to rather recognize the true commitment to service and sacrifices that the law enforcement and public safety community make daily. The entirety of our team, as well as many of you, know all too well the sinking feeling that comes from hearing these stories but not yet the names. Hoping you won’t hear a name you know. I’m sure many of you have also been the recipient of information you wish wasn’t true. Teammates, family members, or friends. 4 heroes were lost today, and others are currently in a fight for their lives. We are praying that those whose fate is still to be determined come out of this and get to return to their families. If you have chosen to be sent, and a path of service whether local, state, federal, or military please take an opportunity to cherish those who also support your calling. Make that phone call last a little longer, hug a little tighter, or those moments you get amongst chaotic schedules a little more memorable. TEAM MED BAGS We love this setup! This team deploys this bag on every operation. A SWAT Officer will have this bag secured to their person. This particular team’s equipment is mostly comprised of black or grey colored items. For their med bag, they opted for ranger green so that it is a different color than the rest of their equipment to make it easily identifiable. This M9 TacOps bag is stocked with: • Tourniquets • Chest Seals • Combat Gauze • Gauze • Triangle Bandages • Warming Blankets • Splints • Rapid Extract Litter • Pressure Bandages • Airway Management Supplies On the backside of the top flap, they stocked it up with some meds such as Advil, Tylenol, Nausea Meds, Rubbing Alcohol, Peroxide, Bandaids, and other medical supplies for those more minor events while on the range, training, etc. Each pouch was labeled with bright red lettering so that there is no mistake on what is where in a stressful situation. Not for eye flushing!!!! If you don’t have Sudecon wipes you can DIY them! If you don’t have anything on hand water, water, and more water! (Flush effected area with COLD water for at least 20 min.) once in a shower use cold water and veggie or olive oil to decontaminate further. Protect yourself when handling contaminated clothing by wearing plastic or rubber gloves. Discard them after use. • Washable Clothing: Wash contaminated clothing separate from other clothes. Cloth- ing which can safely be washed should be washed with laundry detergent in cool or cold water. It may take more than one wash- ing to fully remove the capsicum oleoresin oils. • Non-washable Clothing: Allow non-washable clothing to air dry for a few days. This will allow the oleoresin oils to decompose slightly before cleaning. Dry cleaning can be done once they are air dried for a few days. After drying, other non-washable items can be surface washed with a wet cloth, mild soap and cool water or as the product label instructs for cleaning. Allow to dry completely prior to re-use. • another helpful mixture would be 1 gallon of water, to 2 Tbs of Citric acid, to 9 cups of raw or white sugar, to 21 fl oz of Baby shampoo. [ID in comments]
