In April 2023, forty-year-old Jamie developed extreme body aches that she assumed were due to the flu. However, she soon developed chills and intense abdominal pain that sent her to the ER. “Doctors discovered my uterus and ovaries were necrotic and performed a hysterectomy,” she says. “Afterwards, my condition worsened and streptococcus bacteria was identified. I was given a 2% chance of survival.” Jamie spent months in the hospital and had to have all four of her limbs amputated. Last October, she was finally able to return home from her rehab center. Sepsis Alliance was honored to be in Maryland yesterday alongside the DeSantis family and the Love for Lochlin Foundation as Lochlin’s Law was signed into law by Governor Wes Moore! The legislation was created in honor of five-year-old Lochlin DeSantis, who died in 2020 when the flu rapidly progressed to sepsis. The legislation requires hospitals, urgent care centers and special facilities across Maryland to develop protocols and train care providers to better identify sepsis.
▪️𝟐𝟎𝟐𝟒 - ing 기록 ✏️ 이렇게 밝은 기록은 오랜만인 것 같다 😙 (글에서도 신남이 보이죠??😉) 경구 약제로의 도전은 실패했고, 결국 주사로 매일매일 (사실상 병원에서 집으로 장소만 바뀐) 약을 맞으며 시간과 공간의 제약이 생겼지만, 그래도 이번 두 달 간의 입원을 통해 작년의 최악 컨디션에서 몸이 많이 좋아져 이제 쇼크가 오지 않는다는 것도 알게 되었고, 교수님과 최고의 신뢰관계를 쌓았고, 반복된 실패와 쇼크로 인한 트라우마가 정말 심했는데 트라우마를 깼다는 게 가장 놀랍고.... 다음 도전을 더욱 기대할 수 있게 되었다. - 균도 금방 음전되어서 예정대로 어제 중심정맥관 시술도 잘 끝냈고, 20번 이상부터는 횟수 세지도 않아서 모를 정도로 수많은 중심정맥관 시술로 이미 익숙은 하지만 그래도 약간 긴장했었는데 마침 어제의 담당 시술팀 선생님들 모두 넘 좋으셨구 🥹 (모든 병원을 겪어봐도 인터벤션실은 갑이기 때문에 분위기가 좋기 힘들다) 애기가 너무 많이 한다고 하시면서 위치도 좋은 곳에 잡아주시려 노력해서 어제는 펜타닐을 비롯한 진통제 아무것도 없이도 눈물 한 방울 안 흘리고 시술도 기분 좋게 완료✨ 비록 지속되는 균혈증으로 인한 감염위험과 내 몸 특성 상 항응고제를 복용 중임에도 남들보다 잘 만드는 혈전과, 혈전만 생기면 집이라고 좋아하는 균의 성격 때문에, 외부로 가지고 나가야 하는 것 때문에 사용기간 짧은 C라인 제외하고, 바로 뽑을 수 없는 터널형 (히크만,브로비악,케모포트)도 제외하고, 남은 선택지는 PICC 뿐이지만 그마저도 소중한 걸 어떡해 😌 - 퇴원 전 마지막으로 흉부/복부 엑스레이와 스크리닝 혈액검사 엑스레이도 문제 없고, 낮은 헤모글로빈 수치와 약간 높은 인수치 빼고 백혈구도 호중구도 단핵구도, 심지어 애증의 CRP / ESR까지 모두 정상 👍🏻 오늘 혈구수치 너무너무 조아요~~ - 집에 있는 병원급 의료용품 리스트 다시 체크하고, 처방 받아야 할 것들 체크하고, 밖에서 구하지 못하는데 간호사선생님 말씀으로는 처방이 안 될 것 같다는거 고민고민 하다가 혹시 몰라 교수님께 말씀드렸더니 왜 고민했나 싶게 바로 80일치 챙겨주신 microclave 도 해결!! 이미 Limbo 브랜드 PICC전용 샤워커버 있고, 우리나라에서 팔지 않는데 나는 꼭 써야만 하는 3m curos도 지난 번 집에서 지낼 때 3m 공식 미국직구까지 해서 다 준비되어 있고, 새롭게 mightywell의 PICC 전용 암커버도 구매하고 (우리나라도 의료용품에 관한 편리함과, 예를 들면 빅5중, 아산에서만 쓰는 3m curos 등등의 좋은 것들을 보편화 시켜서 의료의 질이 더욱 좋아지기를) 여러가지로 중심정맥관 관리, 특히 감염관리 이상 무!!! In 2019, while living and working in St. Croix as a marine scientist, Kemit-Amon Lewis contracted a bacterial infection that led to septic shock. After two weeks in a coma, he awoke to the news that doctors would have to amputate his right leg below the knee as well as his right hand, left fingers, and left toes. Over the past five years, Kemit has worked hard to regain his mobility and return to his passions. With help from his doctors at @browardhealth and custom prosthetic care from Florida #HangerClinic team members Brett Rosen and Jesse Mitrani, Kemit is back playing tennis, a sport he's played competitively throughout his life! Earlier this year, Kemit crushed it at the Oceania @parastandingtennis Championships in Melbourne, Australia, and he has his sights set on another international event this summer! 🎾 He’s also enjoying getting back to dance and scuba diving in his spare time. In March, maternal sepsis took 40-year-old Krystal Anderson’s life, just days after the tragic stillbirth of her daughter, Charlotte Willow. Mere hours after the loss of Charlotte, Krystal developed an infection and experienced severe symptoms, including fever and organ failure. Despite multiple surgeries, her condition worsened, leading to her untimely death within four days. “Krystal was a dynamic and impactful person to everyone she encountered,” Clayton recalls. “She thrived in dual careers as a software engineer and a yoga instructor, bringing passion and dedication to both fields. Her involvement with the Kansas City Chiefs Cheerleaders, first as a cheerleader for 10 seasons and later as a staff member, showcased her love for performance and her commitment to spreading joy.” “In memory of Krystal, her family continues to champion the causes she believed in, ensuring that her legacy endures through efforts to advance women in technology and support for those affected by pregnancy complications. ▪️𝟐𝟎𝟐𝟒 - ing 기록 ✏️ 마지막 기록 이후 열은 더 많이 올랐지만 CRP는 오히려 떨어져갔다. 열이 계속 나긴 했지만 부루펜 800mg 정도를 써주면 떨어졌고, 작년에는 열만 나면 답즙토와 혈압저하로 쇼크로 진행되어 파김치가 되었었는데 (교수님이 작년에 너 파김치였다고 하셨다 ㅠㅠㅋㅋㅋ) 그래도 어느새 몸이 견뎌내는 힘을 길러 어떻게든 버티고 있어서 참 다행이라고 하며 위험성을 안고가긴 하지만, 어떠한 증상이 있을 때 응급조치를 할지, 아니면 집과 30분 거리 본원인 아산병원으로 바로 올지, 도와주시는 순천향/삼성 두 병원의 교수님들과도 상의하여 만반의 준비를 한 뒤 수요일 퇴원을 목표로 삼았었다. - 그런데 토요일 오후, 아빠와 동생이 지하로 와서 엄마랑 내려가 넷이 다같이 웃고 떠들고 내가 입원하기 전 ‘그 때쯤이면 갈 수 있겠지’ 하고 잡아두었던 부산여행 일정도 이야기하며 여러 이야기를 나누고 병동으로 돌아왔는데 간호사선생님께서 오시더니 오히려 CRP가 떨어져가고, 열도 많이 나지 않았던 5/10 혈액배양검사에서 균이 자랐다고 하셨다. - 주말 새 컨디션을 주의깊게 지켜봤는데 금요일 혈액에서 균이 자라며 토요일까지는 말도 제대로 못할 정도의 잔기침(마른 기침)을 심하게 했고, 37.5를 한 번 찍어서 (열 올리지 말고 37.5만 되어도 바로 해열제 먹으라는 교수님 말씀대로) 해열제를 먹고 토요일 오후 4시 iv Triaxon, 오후 7시/9시 po Normix,Levokacin을 썼다. 일요일 오전까지 토요일보다는 덜하지만 그래도 심한 마른기침과 오후 37.4의 미열이 있었으나 해열제 복용하지 않았고, 토요일과 똑같이 3가지 항생제를 썼더니 이 날 부터는 체온이 서서히 내려가기 시작하고 기침이 점점 멎었다. 오늘부터는 경구 항생제 모두 중단, iv Ceftriaxon 단독투여를 결정했고, 체온은 올라봤자 37.1, 기침이 거의 아예 멎은 듯할만큼 멎었다. (나는 보통 아예 문제없이 체온을 유지할 땐 무조건 36도대라 37.5가 넘지 않아도 37도 초반의 열도 유의미하게 본다) - 그리고 교수님과 깊이 다시 상의를 했는데 부모님도 나도 교수님도 작년과는 확연히 다른 몸상태에 이번 실패가 많이 아쉬웠기 때문에 교수님께서 한 번 더 시도 해볼래?? 하셨다가 그래도 어떻게 보면 지금의 한 텀 (이번 약 두 달의 입원기간)에서 퀴놀론계 항생제를 두 번 뚫고나온거면 언제든 다시 위험해질 수도 있는 여지가 있기에 위험성을 안고가는 건 무리가 있을 것 같다고 하셨고, 결국은 오늘 내일 혈액검사 상 염증수치와 혈액배양 검사를 확인하고, 균의 음전이 확인될 수 있는 시점인 이번주 목요일에 중심정맥관 시술 일정을 잡았다. (그 사이 음전이 안 되면 더 미뤄지겠지만) - 물론 혈전의 위험성이 다른 사람보다 크고, 혈전이나 카테터에 균이 자리까지 잡아버리면 더 위험해질 수 있는 위험성도 안아야 하지만 무엇보다 가장 다행인 건 지금은 잘 견뎌서 분명 스스로 이겨내는 부분들이 있기에 균혈증으로 뚫고 나오는 횟수가 줄고, 균배양 속도도 보통 2~5일 기다려야 하는 평균과 달리 작년에는 균의 농도가 너무 높아서 빠르면 2시간, 평균이 반나절이었는데 이번에는 배양속도도 27시간으로, 균의 농도가 낮아졌으며, 균도 이정도 항생제 쓰면 더 내성을 만들게 되는데 이번에도 착하게 내성 만들거나 이러지 않아서 매번 똑같이 착하다고 하셨다. ▪️𝟐𝟎𝟐𝟒 - ing 기록 ✏️ 지난주 열이 나며 올랐던 염증수치가 주말 새 38도 이상의 열이 사라지며 함께 내려갔었는데 화요일쯤부터 다시 38도 이상의 열이 오르며 오르기 시작했다. 원래 균이 자라거나 / 열이 다시 오르거나 하면 정주 항생제를 시작하고 중심정맥관 삽입 일정을 잡기로 했었는데 또다시 미루어두고 균이 자라지 않는다면 일단 버텨보기로 했다. 지난주까지는 해열제도 무조건 주사로 맞았는데, 이번주부터는 교수님도 나도 좀 더 욕심을 내어 고열이 아니라면, 균이 안 자란다면, 그래서 집에 갈 수 있다면!! 을 생각하며 집에 갔을 때 열이 나도 경구 해열제로 조절을 할 수 있어야하기에 경구로 시작했다. 첫날은 BRUF200 -> 열이 떨어는지나,떨어지는 속도가 너무 더디고 몸이 힘들어서 어제부터는 BRUF400. 5/7 열 1회 피크 38.4 , 5/8 열 2회 피크 38.3 - 📷 1~5 교수님과 나 사이의 객관적 지표 무언의 약속 열 그래프, 0.6 미만이 정상인데 정상인 횟수가 더 적은 CRP 작년에 열이 나고 혈압이 떨어지며 심부전 수치가 400~800 사이에서 왔다갔다 했었는데 지금은 잘 버텨주는 심장 🫀 - 간헐적으로 느끼는 심실조기수축은 작년 홀터에서 빈도수가 많이 잦지는 않았기에 지켜보자고 했었어서 심해지면 다시 PCD 협진 하기로! ▪️𝟐𝟎𝟐𝟒 - ing 기록 ✏️ 괜찮아진다고 분명 기뻐한 건 아니었는데 입과 손이 방정이라고 (아니 그래도 기록은 해야지....ㅠㅠ) 경구 Levokacin 750mg 의 시작 첫날 새벽부터 열이 올랐다. 연이틀 새벽에 38도를 넘지는 않는 37.8 정도의 열에 이정도면 지켜볼만 하다 싶었는데 어제는 아침에 교수님 회진 다녀가시고 10분 정도 지났을 때쯤 38.7 까지 열이 오르며 극심한 오한을 겪었다. + 어제부터 Normix 400mg도 다시 추가했다. - 교수님과 어제오늘 대화를 나누며 다양한 이야기를 했는데 [그간 써봤던 사이톱신은 시프로플록사신, 레보카신은 레보플록사신인데 둘 다 계열은 퀴놀론계열로 동일. 사이톱신은 클렙시엘라 외에 녹농균 (병원균주) 등을 커버하고, 레보플록사신은 클렙시엘라 외에 마이코플라즈마(지역사회 감염균주) 등을 커버하는데 지금 나에게 나오는 K.pneumonia 감수성 결과 똑같은 750mg 쓴다고 했을 때 그 속에서 상대적으로 적은 용량만 있어도 효과가 아주 약간 더 좋은 건 사이톱신. 그렇지만 퀴놀론계의 두 종류 다 몸에서 뚫고나오는 균을 100프로 커버하지 못하는 게 사실이고, 사이톱신은 세브란스 때부터 매우 여러차례 시도했으나 실패했기 때문에 경구 기준 둘 다 체내 흡수율은 80% 전후 정도로 동일하고, 같은 퀴놀론계 안에서 약을 바꿔보는 시도는 딱히....] 위의 내용과 같이 약제의 선택에 있어서도 수많은 고심 끝에 결정하셨다는 걸 알 수 있었고, 그걸 설명해주시며 언제든 궁금한 건 답해줄테니 물어보라고 하셨다. 워낙 현명하고 똑똑하신 분이신 것도 알았고, 입원전담 교수님들께서도 정말로 지금 아산병원 소아감염과 교수님 (담당 주치의 교수님)의 최대 관심사는 너 하나라고, 정말 고민 굉장히 많이하신다고는 계속 들었지만 한 번 더 직접 정말 나에게 최선의 방법이 무엇인지 찾고 계시는구나 느껴져 감사하고 존경스러웠다. - 그리고 계속 떨어지는 Hb(헤모글로빈)와 알부민, 오르락내리락 하는 WBC(백혈구)와 계속 오르는 CRP, 지속되는 열..... 교수님께서 조심스럽게 이번 시도가 마지막인데 어쩌면 좋은 결론으로 마무리짓지 못할 수 있을 것 같다고. 중심정맥관을 안 잡고도 해볼 수 있겠다라고 더이상 말하지 못할 수도 있을 것 같다고 하셨다. 중심정맥관 시술도 20번이 넘어 중심정맥마저 협착되어 있고, 말초혈관 상태도 당연히 너무 안 좋아 매일 손가락,손목 안, 심하면 손바닥까지 바꿔가며 겨우 유지하지만 지금까지는 의지로 하루에 7번을 찔리고 바꿔도 버텼다면 이젠 정맥 전담간호팀 선생님들께서 혈관 상태가 안 좋아도 안 좋아도 이젠 잡지 못할 정도로 안 좋다고 하신다. - 이제 돌아오는 다음주면 성공과 실패여부는 갈릴 것 같다. 균이 자란다면 당연히 바로 올스탑이고, 균이 자라지 않더라도 계속 고열이 나거나 CRP가 여기서 더 치솟으면 IV Triaxon 2g 정주 항생제로 변경하고 중심정맥관 삽입 일정을 잡아야 할 것 같다고 하셨다. 괜찮아 뭐든 잘 되겠지 뭐!! 하고 마음을 다잡아본다 :) 🤫😃 - 다시 시작한 공부는 너무 재미있다. 오래 쉰 만큼 남들보다 뒤쳐져는 있지만 지금 내 목표는 “치료 유지하면서도 그저 스스로 최선을 다하기” “느려도 괜찮다. 멈추지만 않으면 된다.” 두 가지다. 컨디션도 환경도 맘대로 되지는 않지만 가능한 범위 내에서 최선을 다하면 되지. 6살 때부터 의사, 초5 때부터 외과의사, 중학생 때부터 소아+외과의사 지금은 소아 내/외과 의사를 꿈꾸는 내게 어제는 조용히 “넌 뭘 해도 잘할거다.” 하며 존경하는 교수님께서 한 마디를 던져주고 가셨는데 따뜻하고 감사했다👶🏻🩺 오늘도 휴일인 토요일에 현재 병동에 교수님 앞으로 입원한 환자는 나 하나 뿐인데 출근하셔서 회진을 돌고 가셨다. (매주 토요일까지 출근하셔서 보러 오신다) 엄마와 나와 많은 대화를 나누고 허리 아플 것 같다며 교수님 방에서 커블체어를 꼼꼼하게 소독까지 해서 가져다주셨다. 오늘도 감동🥹🥹 나도 환자의 마음까지 품어줄 줄 알면서도 교수님처럼 현명하고 똑똑하며 (교수님 서울대 의대🙄) 세심한 의사가 되어야지. Today I tried to get them to take a picture to show off my new @WNBA orange tee but they wouldn’t look at the camera! #GoldensOfInstagram My energy level fluctuates substantially day to day but it is great to be back at home and even visiting my favorite spring stomping ground - @ritasice in my #HandleHardBetter shirt (check out @karalawson ‘s speech to @dukewbb if you have seen it!) I was also able to stop and get the family sorbet (sense a trend!) from @gabbyssweettreatsdmv who came to Kentlands! I even had enough energy to pitch a tent (with a lot of help from Aubrey!) and hang out at the #Kentlands #CampOut last weekend. It was a blessing to just sit at a camp fire and relax and talk with friends. My recovery did take a small step back as we think I have an inguinal hernia that is causing some pain. I can’t get in to see a specialist until the end of May, so I have to live with the pain right now and modify my PT. I’m also still being treated for the blood clot, but all the infections appear to be gone. Gail dealt with digestive system issues throughout her entire life. In March 2020, she was experiencing a flare-up that began to get progressively worse. She insisted to her husband that she could ride it out and wait for an appointment with a GI specialist. “After fighting this on and off for two weeks, she suddenly began to have shortness of breath and was lethargic most of the time,” her husband, Peter, recalls. “Her body temperature began to fluctuate and then suddenly she began showing signs of brain fog. At that point, Gail finally agreed to go to the emergency room.” In the emergency room, a CT scan revealed that undiagnosed diverticulitis caused Gail’s colon to rupture. A bacterial infection had since developed and progressed to sepsis. Gail was now in septic shock and her vital organs were shutting down. “Gail’s death is a stark reminder of why the acronym T.I.M.E. is so important in the fight against sepsis,” Peter shares. “When this episode occurred, it didn’t go away so quickly. I kept insisting that we get her immediate medical attention, but going near an emergency room was not what she (or many others) wanted to do at a time when COVID was beginning to spread through the country. I’ve been speaking to a lot of Sepsis Survivors recently and one thing I’ve noticed is that a lot of the signs and symptoms of one thing can be the same as another, and sometimes we focus on just one thing when there’s possibly lots going on. Remember: you’re not just a Sepsis Survivor, you’re a human. You can have more than your ‘recovery’ going on. 🥗 Maybe you’re not eating right and that’s causing some symptoms to become worse. 😢 Maybe you’re isolating and the lack of human connection / fresh air / sunlight / movement is making things more difficult to deal with. 🧬 Maybe your hormones have taken a hit and you just can’t be bothered keep up with self care … or your anxiety is so bad that you can’t think straight. 🦠 Maybe the infection that caused the sepsis is still there and causing you problems. Today is my alive day and I almost forgot! I was driving the kids to school and it dawned on me…8 years. It’s been 8 years since my life forever changed. Septic shock, amputations, and skin grafts were my initial changes, but over the years my loss has evolved. There are days filled with sorrow, lots with joy, almost always frustration and all the other emotions that are a part of being in this world. I know that I am thankful that I survived. I know that I am thankful that I am allowed to be witness to my children growing older and to be a mother and a wife and a friend. ▪️𝟐𝟎𝟐𝟒 - ing 기록 ✏️ - 글을 기록했던 지난 주 금요일, 복통이 생각보다 많이 심해져 정규시간에 문제가 될만한 약들을 hold 했지만 당직시간에 당직 교수님께서도 오시고 진통제도 5번 이상 간격만 되었다 하면 계속 썼었다. 그리고 Normix를 중단하니 주말에는 50% -> 20% 순으로 좋아져서 비교적 편한 생활을 했다가 일요일부터 Normix 800mg -> 400mg 로 줄여 다시 시작했더니 월요일에 또 극심한 복통, 다시 hold 하고 어제는 진통제 한 번 투약으로 끝나고 오늘은 괜찮다. - 항생제 투약에 대해서는 보통 혈액에서 균이 자라면 정주 항생제를 쓰는 ‘최소’기간이 5일 정도, 길면 2주 (상태에 따라 쇼크나 혈전 등이 있다면 더 길어짐) 인데 다시 마지막으로 경구 항생제의 시도를 위해서는 정주 항생제의 최소기간만 지키고 다시 경구로 바꿔보자고 하셨고, K.pneumonia 의 경우 그람양성이 아닌 그람음성균이며, 나를 작년에 열 42도, 혈압 60/30, 혈액응고수치는 너무 높아 측정불가, 염증수치 정상의 50배, 세균감염수치 정상의 70배 등등 매우 위험하게 쇼크 상태로 만들었던 균이기에 일주일은 써야 한다고 하셨었다. - 그렇게 일주일이 벌써 흐르고, 오늘부터 새로운 도전을 하게 되었다. IV Triaxon 2g (4/23~4/30) 오늘부터는 Levokacin 750mg 단독투여, 그리고 반응을 지켜본 후 Normix 추가하는 수순으로 가자고 하셨다. (아마 복통의 원인일 약이라 정말 쓰기 싫고 힘들었지만 그래도 반응을 보려면 2주는 쭉 써봐야 알 수 있다고 하셨고, 노르믹스를 추가하게 된 이유가 레보카신의 성공을 전제로 했을 때 레보카신+노르믹스 조합으로 예방적/치료적 경구 항생제 장기사용을 위한 것이라 시도해보고 싶다고 하셨다) 치료를 이어가기 위해서는 약에 대한 환자의 순응도가 좋아야 하기 때문에 복통이 심해지면 다른 약들도 먹을 수 없어져 쓰기 어렵지만 노르믹스도 마지막으로 한 번만 더 시도해보기로 했다. (일단 레보카신에 대한 시도부터...) 그리고 배제할 수 없는 실패에 대한 차선책으로 중심정맥관 삽입에 대한 준비를 해야하기 때문에 목정맥에 남아있는 혈전을 평가하며 겸사겸사 수십 번의 중심정맥관 시술로 중심정맥마저 대부분 협착이 된 혈관 상태를 확인하는 초음파 일정을 내일로 잡아두었다. - 산뜻하고 푸르고 맑은 5월의 시작에 새로운 시작을 두 개 하게 되었다. 컨디션이 좋을 때 써두려고 기록하느라 시도 첫날인 오늘 컨디션이 어떻게 될지는 모르겠으나 체온은 정주 항생제를 유지하는 동안 36도대를 쭉 유지하며 4월 이후 처음으로 염증수치도 정상이 되었는데 오늘부터 체온이 37도대 초반으로 올라왔다. 1. 이번 경구투약 시도가 마지막이다. 제발 성공해라 🙏🏻 사실 작년과 같은 상황이면 다른 게 문제가 아니라 당장 생명이 왔다갔다 할 만큼의 상태이기에 아무것도 할 수 없었는데 올해는 그나마 비교적 안정적인 상태가 되니 다시 공부에 대한 욕심이 올라왔고, 드디어 몇 년간 쉬며 눌러두던 의욕을 꺼내보기로 했다. 한참 전에 기록하던 공부기록용 계정의 모든 팔로워를 지우고 생애 처음으로 인강이라는 것을 알아보고 결제했다. 그리고 하나의 새로운 글을 올렸다.
