The insurance based, PT Mill style of care has resulted in physical therapy becoming personal training sessions dressed up as physical therapy. There are many problems with this. 1) There is not very much education and training in physical therapy school specific to exercise, strength, and conditioning. 2) It devalues the profession, because patients view PT as only sitting on an exercise bike and doing rubber band exercises. 3) Physical Therapists aren't allowed to use the higher level skills they learn. 4) Patients suffer because they are receiving exercises from providers that are most likely lacking high level exercise physiology or strength training knowledge. As a consumer of Physical Therapy you can help. You have a choice in where you receive your care. Take some time to search out providers that use there full skill set, and don't just watch you exercise. If you do this you will be amazed at how much better your experience and results are. If you are a Physical Therapist, stand up for yourself. You are more than a personal trainer. If your boss won't let you use your full skill set seek out employment at a clinic that will. The student debt to income ratio is going to kill the profession of Physical Therapy if things don't change. The APTA pushed to get all Physical Therapists to become Doctors adding a year to school and increasing the financial burden of becoming a DPT. But we continue to see cuts to reimbursement. This makes it impossible for clinics to pay their clinicians a salary on par with the education and expertise of the profession. We are seeing PT's having to take on extra jobs just to make ends meet and make payments to their student loans. The results of this are burn out and leaving the profession. If everyone had to write a check each month to their insurance company I bet things would change. Instead your premiums are "conveniently" taken out of your paycheck before you even get paid. This causes you very quickly to just "forget" about how much it's costing you. This also leads to devaluation of healthcare professionals. Because the premium was conveniently taken out of your paycheck and since you only have to pay a small copay at appointments you don't see the true cost of your appointment. This leads to a subconscious view that the expertise and knowledge of the healthcare provider is not very valuable. Meanwhile your insurance company continues to raise premiums every year while reducing reimbursement to the professionals delivering the care. You hear about the increased premiums and are upset for a minute, and then forget about it because the money is taken from you before you get it. Insurance companies aren't trying to improve your health, but they have perfected how to keep taking your money without causing outrage. With the "PT Mill" style of Physical Therapy everyone loses except the insurance companies. What is a "PT Mill"? PT Mills are the most common form of outpatient Physical Therapy clinics. This is where the Physical Therapist is juggling 4-5 patients at the same time and passing off most of the care to assistants and aides. How does this result in everyone suffering except the insurance companies? Here's how. If a PT is required to juggle 5 patients an hour at the same time that means they are handling 30-40 patients each day on top of all the necessary documentation and paperwork that insurance companies demand. They receive very low pay (more on that to come) for the amount of work required. This leads to the high levels of burn out we are seeing in the profession. We are seeing a trend in PT's leaving the profession after 5 years when they've had enough. Patients suffer because they are spending a majority of appointments receiving care from less skilled and qualified providers, which results in them wasting time and being required to come back for appointments more frequently. Insurance companies prosper in this style of care because they know the level of skill is less, so they continue to cut reimbursement rates adding to their substantial profits every year. It's time to stand up against this type of Physical Therapy model. If you are a patient, seek out clinics that are not PT Mills. If you are a provider, search out a clinic to work for that is not a Mill. On Tuesday 4/23 the FTC voted to ban all noncompete agreements and made previous signed noncompetes invalid. Like everything in the government it is probably going to get tied up in court. But it is a step in the right direction and should give you power to get out of the soul sucking PT Mill job you are stuck in because of a noncompete agreement. It's time to get out there, find a job that puts you and your patients first. They exist you just have to look. The APTA tries to act like they are helping the PT profession by "fighting" for increased reimbursement. It sounds good at first, but if we take a closer look it is probably a losing battle that isn't worth it. Since 2021 Medicare has cut reimbursement for Physical Therapy by 13.4%, in the same time frame the buying power of the American Dollar has decreased by 18%. So just to get back to a net zero from 2021 we would need a 31.4% increase in reimbursement for Physical Therapy. Let's get real for a second and understand that isn't going to happen. Instead of fighting a losing battle for reimbursement that won't improve our situation we should show our value and stop allowing ourselves to be devalued by the insurance companies. The common saying is $#!+ or get off the pot. I've been a quiet "complainer" for far too long, and have had enough. I have decided that I am going to TRY and help save the physical therapy profession. We are at a crossroads, and we Physical Therapists have a decision to make.
