what is a standardized patient? an ‘SP’ is an “actor” basically. we are given cases to present to medical students during OSCEs or doctoring sessions. they can range from abdominal pain to dizziness and it’s a way for medical students to practice before they get out in the real world. with the name ‘standardized,’ it means that all people working as an SP need to be the same. meaning have the same story. we train for these cases and go through them piece by piece to make sure all of us are on the same page. medical students will perform exams on us too! they get to practice how to do abdominal exams, neuro exams, and MSK exams, etc. sometimes we get make up to display a sprained ankle which helps the student understand the severity of the case. as an SP, we can also grade. we have a checklist of items that the medical student should be doing. for this, we are in a room and watch the OSCE live and grade them as it happens. I think it’s such a cool experience that I get to add on my CV and application! Application series: Part II One of the most common questions I see asked is: “Which premed job/clinical experience should I pursue, ___ or ____?” When I was looking for a post-grad job, my main priority was finding something that would allow me to be immersed in what I was interested in. I was very fortunate as the job kinda came to me. Retrospectively, I think it was more like fate. Now, I get to hang out with physicians and medical students all day that love to teach and have the best advice. As a scribe, I have learned so much clinically that I feel I could not have in any other typical “premed job”. Many medical students have told me they are so happy they chose to scribe (or wish they had) bc you learn so many applicable skills that are useful in medical school and beyond. This is not to diminish other common premed jobs, just sharing what I have heard from medical students! Of course, with scribing you don’t examine the patient or even directly speak to the patient for the most part. This is one of the reasons why I also worked as a dementia caregiver in order to familiarize myself with “direct” patient care. I want to also say that when I started out, I had absolutely no connections to medicine. At times it can be difficult to “get your foot in the door”, however, once you do, many doors will open for you. You just have to start somewhere! ⚡️ HOW to scribe FASTER⚡️ when I was in training, the scribe who was training me expressed I needed to finish charts quicker. To her credit she was absolutely accurate. But it’s really hard when you start a new job field and people are telling you do things quicker when you barely grasp what you’re supposed to be doing. So if you are like me and have been told to to scribe quicker but don’t know how this post is for you. Mind you I don’t need this list anymore I only needed it for like the first month and a half and I used a list to make sure I didn’t forget any aspect of the note. As you move on it’s second nature it’s like a mental checklist in your head that you just know what you’ve done already. ⚡️ Pre- chart - if I’ve forgotten to pre chart of didn’t get to it due to various reasons (provider coming in early my computer not starting ect) ⚡️ HPI: making sure my HPI is as clear and readable as it can be and making sure everything is spelled correctly & I’ve looked up all information I needed ⚡️Imaging: If a test was being run I didn’t check this off until it was completed and entered into the chart ⚡️ Tactical/physical exam: Making sure I had the names of areas correctly and made changes to the exam from last visit ⚡️ Assessment: making sure I talked with the provider if the diagnosis had changed ⚡️Plan: making sure I got all aspects of the plan they discussed So if I didn’t complete something I would circle it and if I did complete it I put a check by it and that prohibited me from having to constantly open 15 different notes throughout the day and check every aspect of them. It’s sped things up because I knew which notes were already done and which notes needed to be completed. At the end of the day if I had a question for provider it wasn’t taking me 20 minutes to find the question of the chart I had which helped both of us finish our tasks quicker

⚡️ O B J E C T I V E ⚡️ Swipe ⏩ ⚡️ PHYSICAL EXAM: One providers does not say anything about the exam in the room but after he will dictate extensively so I have to be ready when he talks about it. My other provider will one or two things while in the room and I need to listen for those. When my provider is not saying anything to me I will edit my note ⚡️ LABS: prior to me going into the room I’ll make sure the labs are already in the note every once in a while a lab result will not be back before going to the room. I’ll look to see what appointments are scheduled for that patient for that day so I can see what test they’re having done. My providers use my note with the test results to show the patient in the room so if I don’t have them in my note it will prolong patient experiences. ⚡️ PATIENTS: I have heard patient say things while getting the physical exam that is important so it’s important to use your time wisely but not completely tune out from with a patient saying during the exam. ⚡️ FINDINGS: it’s extremely important to document the physical exam correctly because if there’s a abnormality on a test and we didn’t document on the physical exam findings it’s extremely hard for insurance to want to bill for that. Which can be a issue for patients and will make them possibly not want to seek care again. Pro- tip: if you can start to pick up where are your provider usually looks for issues it’ll be easier for you in the long run so for example in my pulmonary clinic it’s mostly the cardiovascular system the pulmonary system and and from there she will check for Adema that’s important you understand where they are in their exam and how it goes so when they say some thing you know what you’re documenting. Pro-tip: make sure to change findings that aren’t relevant to the current visit Our job is as a scribe is extremely important and vital to the patient care being documented correctly because if it did not there’s not a document and it did not happen. Remember knowing your provider preferences is key! YOU GOT THIS

⚡️ SUBJECTIVE/HPI⚡️ Hi y’all 👋🏻👋🏻 Hopefully you had the best thanksgiving & were nice to yourself because you deserve that and SO much more! Since last post relating to this was an overview I want to go into how I deal with the subjective portion of a note - reminder every provider is different so this may not relate to all scribes ⚡️Subjective portion of the note is the part that is the most detailed and contains the most content (for a scribe) So to start for returning patients I’ve already pre-charted and pulled x-rays/MRI I’ll only add testing to the HPI for one provider so again know your provider preferences. it’ll go something like this Ms Jane Doe is a pleasant 52 year old female returning for her 6 month follow up. Since last visit, her symptoms have gotten better and are manageable. She is currently maintained on “enter medicine here”. Diagnostically she had a MRI of lumbar spine on 11/11/21. ⚡️For new patients you can’t do much for pre charting you can pull a template and glance at outside records and see if you can make sense of it but some outside records are incredibly hard to read. Depending on your provider but most times it’ll just get dictated to you. So the things I listen for returning patients are if they’ve gotten worse & onset “ I’ve had a cough for a few weeks and it isn’t getting better” ⚡️Always listen for the provider prompted questions “ have you tried anything for it” “DayQuil” Then I’d type “ her symptoms have gotten worse with an accompanied productive cough. She recently has tried DayQuil with no benefit” ⚡️Most times the story will be told out of order and may change so make sure you don’t tune out after you have your answer because it may change later on! ⚡️So to wrap up important key points are: - onset - Location - Duration - Type of pain - What makes the pain worse or better - Any other symptoms - What has been tried to help with symptoms - If they’ve been seen at an outside clinic I SO hope this helps more to come! 🖤🖤 Any questions you had as a new scribe that you felt weren’t answered during your training?

⚡️ OVERVIEW OF SOAP NOTE⚡️ I debated expanding on this note more but I wanted to the ability to really get in depth in each section which one post didn’t allow me the ability to do! So first in a very long series 😂 Subjective: I listen for onset of symptoms if there was any recent fall of injury that could’ve caused such symptoms. What they’ve done to treat symptoms in the past etc Objective: This is where pre charting is helpful before I go into the room I check and see what previous imaging had been done and make sure it’s added in the chart Assessment: This is provider based, some providers tell you verbally and others will dictate this to you. I just make sure there is a diagnosis before I leave and if there isn’t I just ask the provider what they would like to put Plan: Most providers will discuss the plan of action in the room so make sure you’re listening for it which usually includes: follow up, medication, imaging, labs and tests and return visit. Some providers do recap at the end or tell you later so again make sure you know your provider I so hope this helps any new or future scribes right out of the gate!!

⚡️ PROS OF SCRIBING⚡️ When I got my job as scribe it was a very black and white mind frame. -How is this going to help ME? Now after some time on the floor and provider interaction it’s how am I helping with assisting with patient care? Getting a job in healthcare is humbling because you find out QUICKLY if you are cut out for it. I absolutely LOVE it. It’s exhausting but so rewarding. I get asked quite frequently in my personal life what is the point of being a scribe so instead of listing off my duties (which you can find on google) I thought I’d express how my job helps the provider and helps patient care go smoothly. I want you to remember no matter how small your job you are making a difference in someone’s life. If it’s picking up a phone to schedule an appointment that could help someone feel better. Or taking someone’s vitals that helps decide if they need a certain type of medicine. Or if you are documenting their symptoms. We ALL are important. We help the provider who then helps the patient which is the main goal. I felt guilty for being so determined on my goals I forgot for a second the real goal here, you know what I realized? It’s okay we are human. It’s why it’s important to have jobs that remind you daily and make it all worth it 🖤🖤

⚡️POST 12 HOUR SHIFT VIBES⚡️ Hi lovely humans how are you?! This photo was taken directly after getting off a very long shift. I always like to level with you the transition has been HARD but worth it. I thought my life wouldn’t change I’d just be working more. My shifts with my provider move at incredible speeds with really no breaks for food water or otherwise unless we have a break or until they are done for the day. On an average day I walk 8 miles. When I get off I exhausted, starving and thirsty which I’m working on adjusting. It really means my friends will text me amazing news or send me pictures and I don’t respond. Not because I don’t care but because I don’t have the mental capacity to hold a conversation lately. So to the new person who is the first one in and last one out and still feels as if it isn’t enough I hear you. Yesterday I had a PRETTY rough day. Getting all my mistakes on a chart pointed out was hard and in front of other people was even harder. So it’s hard when you are doing your best and it’s still not enough. But it will be. You’re sacrifices will be worth it I promise. You will get a routine down. Your friends will understand. You won’t feel like you are downing eventually. My schedule looks like: - Up at 4:30-5:30 - Drive to clinic from 5:30-6:30 - done with the day from 4:30-7:30 - home by 5:30-8:30 It varies but most days I get home around 7 and get ready all over again for the next day. But things do have to change. After 12 hour shifts I don’t do much I get ready for bed. Fortunately Chris makes dinner and grocery shops most of the time because I don’t have the mental stamina these days to do those small tasks. It’s okay and the farther you go into this journey the more you’ll have to delegate. That’s normal you aren’t supposed to do everything. I’m so proud of you. You got this.

⚡️ INADEQUACY ⚡️ Hi all! As you know I started a new job in a new field! So I’ve been feeling VERY overwhelmed lately and that’s to be expected. Here are some things that have helped me find my footing in this new time! ⚡️ WORKING TOWARDS YOUR GOAL - There is SO much to learn with scribing. I’d go home and work towards something I learned that day if it’s a term, a drug or a diagnosis so I made sure I was actively trying to learn at home as well and it made me feel better knowing that for the next time it comes up. ⚡️REFRAMING- This is a life long journey but reframing my whole I don’t know anything to I don’t know “x” topic yet. Then when you do know it coming back to it and realizing you aren’t supposed to know everything off the bat it’s called training for a reason ⚡️SMALL GOALS- I love scribing but doing a whole note is overwhelming at first so instead of making one big goal to complete I made them into smaller goals. so I found one thing every day I could complete. When I first started out it was catching the chief complaint and 2 details from the HPI. Make small obtainable goals so you don’t feel like you aren’t making progress ⚡️ DOCUMENTING- It’s SO easy to feel like you haven’t progressed when you don’t have it documented anywhere. I started journaling my hard days with a concept and the good days so I can look back when I’m having another rough day with a different concept, and realize that I will get this it just takes time. ⚡️ WANTING TO LEARN- When you don’t know something it’s scary to step up to the plate. But, that’s exactly what helped by taking initiative and asking to do more. Sure you get a lot of it wrong at first but you are taught how to do it next time. I asked for more responsibility so I could learn more and the more you do it the better you get. It’s okay to not know everything, everyone was new and didn’t know anything either once. ⚡️ Life begins at the end of your comfort zone⚡️ - Neale Donald Walsch This ride isn’t perfect but it’s worth it. I’m exhausted physically but mentally? I feel like I’ve won the lottery Keep pushing I believe in you ⚡️🖤🤍

⚡️ PRE- MED JOBS⚡️ Hi all sorry for the lack of informative posts it’s been a crazy week training! There are a lot of pre med jobs out there and it was pretty hard for me to decide what type of job I wanted but there are some of the things that helped me, hopefully they help you! ⚡️ TRAINING TIME: this was a big one I was in a busy time when I applied so I didn’t really want to take a semester off to have to take a certification class. ⚡️ SCHEDULING: how flexible was the scheduling? School is my first priority and I know some people can handle a full time job and full time school but that’s just not my vibe. ⚡️TYPE OF HOURS: I really wanted shadowing hours for a number of reasons but my main one was to make sure this is what I wanted, and to gain insight into what an encounter with a patient looks like ⚡️PHYSICIAN INTERACTION: I really wanted a job where I had quite a bit of interaction with the physician vs the patient. ⚡️ RESPONSIBILITY LEVEL: I wanted a job where my job made a difference but wasn’t a ton of responsibility and I didn’t feel like the world would end if I made a mistake These are things I thought about while dipping my feet into the job pool of healthcare, I absolutely made the right choice! Another post will be coming soon of what importance does a scribe play Right now school isn’t going great so once im out of the school storm I’ll update you on that as well!
