Emisia, jako alergik, miewa zapalenia zewnętrznych kanałów słuchowych. U takich pacjentów najczęściej mamy do czynienia z nadmiernym namnożeniem drożdżaków. Ten psiak jest idealnym przykładem na to jak istotnym badaniem jest cytologia. Dzięki zbanianiu pobranego podczas otoskopii* materiału mogliśmy wprowadzić leczenie celowane. Cytologia pozwoliła na uwidocznienie bakterii. Emisia przez to musiała mieć wprowadzony inny lek niż przy typowym drożdżakowym zapaleniu. Od zdiagnozowania alergii i wprwadzenia odpowiedniego leczenia Emi nie miała nawrotu. *Otoskopia to oglądanie kanału słuchowego przez specjalną kamerę Na drugim zdjęciu - cytologia przy drożdżakowym zapaleniu kanałów słuchowych. This goose presented with peri-orbital sinusitis, an inflammation of the sinuses around the eye. It showed signs of nasal discharge, lacrimation, and difficulty breathing. I aspirated the sinus fluid to examine it under the microscope, revealing a heterophilic inflammatory response but no infectious organisms. While Chlamydia was a possibility, a culture was declined by the owners. Unfortunately, the goose was in a late stage of the illness and didn't survive despite treatment attempts. This case highlights the challenges of diagnosing the cause of sinusitis without a culture and the importance of early intervention in such cases. Foliculitis bacteriana superficial en bulldog francés 🐶 🗒️ La mayoría de casos es causada por Staphylococcus pseudintermedius, aunque puede haber otras especies de staphylococcus u otras bacterias involucradas. Estas bacterias ingresan debido a traumatismos locales, rasguños, seborrea, demodicosis, factores hormonales o alergias. 👉 Es importante tener en cuenta que la foliculitis puede ser pruriginosa o no, su intensidad puede variar, lo cual podría deberse a la hipersensibilidad bacteriana individual. Just because a patient has a history of pemphigus foliaceus... doesn’t mean every crusted lesion is an autoimmune flare. This pet seemed to flare with crusted lesions as I have been managing his pemphigus foliaceus. This definitely could be a pemphigus flare. However, we always check a cytology because other things can cause crusted lesions on dogs too. Like pyoderma! That is exactly what was happening in this dog (check the picture of his cytology). If I had just increased his steroids, things wouldn’t have gotten better (in fact, they could have gotten worse). Papillomas can be seen due to immunosuppression. We see them in really young dogs who often grow out of developing them with age. You can also see them in older dogs. However, some medications can pose a small risk as well (steroids, cyclosporine, Apoquel, etc.). So, what do you do if they develop? This dog was recently placed on cyclosporine and I noticed this small papilloma on the lip during my exam. Upon asking the owners, the dog has developed a small papilloma previous to the medication. Cyclosporine has dramatically improved this dog’s allergic dermatitis so we elected to continue the medication. Cryotherapy was used to help get rid of the papilloma and they were instructed to monitor for any more. In severe cases of papillomas, I have discontinued the medication. You can also try reducing the dose or frequency. Just like anything else in dermatology... It depends. Pentoxifylline is traditionally the hallmark treatment for cases of cutaneous vasculitis. However, some cases need other medical interventions. This sweet girl presented on pentoxifylline and had already failed doxycycline/niacinamide and steroids for her vasculitis. We discussed the options of cyclosporine, Apoquel, tacrolimus, etc. The dog also has seasonal allergies so we decided to see how she does with Apoquel. Now that we have had the medication for over a decade, we have learned some uses besides allergic dermatitis it can be helpful with managing. Vasculitis is one of those diseases. I checked to make sure there was no infection with cytology, increased the dose of pentoxifylline (since it was on the lower end) and started Apoquel. I will re-evaluate her in a month to see how she is doing. I usually keep them on BID dosing until recheck before deciding to attempt SID dosing. Just like any other medication, it won’t work for every case. However, I have had some great success in some cases. 🔥Nocardiosis in cat 🔥 🔸Cat, mixed-breed, male, 7 years old, has an outdoor access. He has slowly developing (during about 6 months) nonhealing lesions of the subcutis and skin of the abdomen and groin without pruritus; another parts of the body are normal. 🔸The lesions were presented by draining tracts, nodules with sero- to sanguinopurulent exudates and ulcers. 🔸Activity and appetite is slightly reduced in the last month. Cough, vomiting, diarrhea, fever were absent. Oral cavity is normal. Feline respiratory infections were in the past. 🔹The differential diagnoses included Mycobacterial infection, Nocardiosis, Actinomycosis, Pseudomycetoma. ▪️Cytology (FNA): Pyogranulomatous inflammation (neutrophils, activated macrophages). There were only a few neutrophils with phagocytosis of rod-shaped bacteria / bacterial filaments. (It was very hard to find them, only 4 neutrophils in 10 glass slides, practically all cytologic samplings looked «sterile»). Mycobacteria are absent. ▪️Histopathological examination (Irina Bogomolova): pyogranulomatous dermatitis, panniculitis. Splendore–Hoeppli phenomenon (asteroid bodie) – skin infections, including «rare» skin infections, are suspected. ▪️Abdominal ultrasound: without pathologies. ♦️A diagnosis of nocardiosis is usually suspected based on clinical symptoms and the presence of filamentous / rod-shaped bacteria, as in my case with this cat. Confirmation of the diagnosis requires culture or PCR. ♦️Nocardiosis is very difficult to diagnose: negative culture or histopathological examination are not exclude nocardiosis. Topical therapy can be absolutely amazing! Culturing this sweet dog’s neck showed a very resistant MRSP. While we we were waiting for the culture results, I had the owner bathe frequently with a 4% chlorhexidine shampoo and apply mousse when she could not bathe. I called the owner to discuss options (which could have included labwork and scary systemic antibiotics like rifampin). The owner told me that the dog had actually been doing much better with topical therapy alone and she was committed to continue it. Music to my ears! We forego the scary antibiotic with potential liver side effects and focused on allergy control with topical products for infection. Amazing to see what topical therapy alone can do when we rechecked! Don’t be afraid to focus on topical products with these cases! Cutaneous TVT in a dog, Wright’s staining. Canine Tranmissible Veneral Tumors(TVTs) are cauliflower-like, pedunculated, nodular, papillary, or multilobulated in apperance. Cutaneous TVTs are occasionally seen on skin and in oral, nasal, and conjunctival cavities. The tumor is transmitted from site to site and from dog to dog by direct contact with the mass. Chemotherapy/radiotherapy is the treatment of choice. In cytology, these cells are distinctive discrete round cells. CYTOLOGY EVERYTHING!!!!! Even as a specialist, I get fooled by skin lesions! This is an English bulldog I manage for chronic atopic dermatitis and food allergies. His management has been very tough and he has struggled with recurrent pyoderma. When the owners sent me pictures of his lesions, I encouraged them to come in because I wanted to perform cytology. But I was convinced it was going to be a bacterial folliculitis. Well, I was SHOCKED to see the biggest yeast party I have seen on skin in quite awhile. Even rivaling some of my elephant skin Westies! Nobody has microscopic eyes and it is so important for us to appropriately diagnose these cases through cytology.
