A 28 year old male patient, known case of CKD on hemodialysis, presented with swelling in the left groin. X ray pelvis shows a well defined expansile lytic lesion arising from the left superior pubic ramus, with areas of ground glass density and a narrow zone of transition (green ring). Rest of the bones show altered bone density with osteopenia. Widening of the sacro-iliac joint spaces is seen bilaterally with subtle sclerosis (white arrows). These findings are in favour of a metabolic bone disease. NCCT confirmed the findings, with the lytic lesion showing multiple septae, and blood-fluid levels with areas of ground glass density (yellow arrow). The SI joint spaces show subperiosteal resorption with osteopenia (white arrows). MR showed areas of chronic hemorrhage and multiple fluid fluid levels within (blue arrow). Mild post contrast enhancement seen in the ground glass segments (yellow arrow), likely fibrous matrix. Areas of blooming on GRE images are seen, suggesting chronic hemorrhage (green arrow). Based on the imaging findings and clinical history, we reached a final diagnosis of secondary hyperparathyroidism (HPT) with a brown tumor. Serum PTH levels were markedly elevated 1000 pg/mL (normal 10-55 ng/mL). Secondary HPT in CKD results from hyperphosphatemia, which stimulates parathyroid glands. This results in the radiological features of hyper PTH, including subperiosteal bone resorption - predominantly at the radial aspects of the mid phalanges of the 1st and 2nd digits, lateral ends of the clavicle and sacro-iliac joints (resulting in pseudo-sarco-iliitis appearance). Bone changes in CKD are termed osteitis fibrosa cystica, which includes - Brown tumors which are the localised form of the disease. Brown tumors are characterised by rapid bone loss, areas of hemorrhage and laying down of granulation tissue, which will result in multiple fluid-fluid levels and areas of hemosiderin staining on MR imaging. Learning point is to consider changes of renal osteodystrophy in any patient with CKD, the changes of which can range from osteopenia to osteosclerosis to lytic lesions. #radiology #neetpg #radres #radresidents #orthopedics #skeletalradiology

Today was a day to take a breath and reflect. My dear friend Prof. Iris Eshed from Tel Aviv in Israel was in Berlin on her way to the #ESSR2022 congress of the European Society of Skeletal Radiology in Rostock-Warnemünde. We did what Berliners do: had breakfast for hours in the middle of the day as if there were no tomorrow. I had an intensive collaboration with Iris at the Charité @chariteberlin while she was working with us as a research fellow. During this time, we wrote many papers that contributed greatly to my habilitation (Venus legendi - a PhD more or less). Even in the time after that, we repeatedly evaluated material together and published papers. It is wonderful to be able to rely on friends, even if they are far away. What is it like for you? Have you ever wanted to read 10 pages about an MRI criterion? Surely not! But the ASAS definition of a positive MRI of the sacroiliac joints, unfortunately, offers a lot of food for thought, because its application is often simplified or in the wrong setting. Many false-positive findings are the result. Many patients receive therapy for axial spondyloarthritis that they do not need. Much criticism is leveled at the ASAS MRI criteria because the definitions are applied carelessly. Isolated teres minor atrophy can be from a quadrilateral space mass however in a large majority of isolated teres minor atrophy no mass can be identified in the quadrilateral space and quadrilateral syndrome is a rare cause of teres minor atrophy. ´ The anatomical relationship of the teres minor nerve to the joint capsule and the frequency of associated shoulder injuries in patients with teres minor atrophy raises the possibility of an association between humeral decentering and teres minor atrophy’. It was a great pleasure for me to contribute to the musculoskeletal radiology training week of the German Society for Musculoskeletal Radiology DGMSR this year. Together with Dr. Sandra Hermann and Prof. Torsten Diekhoff, we covered the topic of imaging in rheumatology as the "Berlin Group". Almost all lectures are now completed, but the training week continues till Friday. Everything from rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis to gout was covered and thanks to the clinical keynote lectures by Sandra Hermann, radiologists in the audience were able to get a good grasp of the relevance of the various imaging modalities.
