Recent HIV and Treponema pallidum infections. 🔻CT findings: Multifocal thoracic and abdominal aortic aneurysms (mostly saccular) with pseudo-aneurysms and mural thrombus, extending from the aortic arch to the right common iliac artery. Extensive aortic wall calcifications. Aneurysms involving the coeliac, SMA, and IMA origins. Aortic root, ascending aorta, and renal artery origins are spared. 🔻Luetic aneurysms, caused by syphilitic aortitis (tertiary syphilis), result from chronic aortic wall inflammation. Our patient is a 17-year-old female. Several weeks prior to her presentation she started to experience right hip discomfort and eventually pain which gradually progressed. Her ambulation was limited to 10-15 steps at that time. A mass was palpable in her right gluteal region. X-rays were done, which demonstrate an osseous mass originating from her right iliac bone. Margins are lobulated and there is an intact cortex. Subsequently she underwent a CT (swipe), which showed this osseous outgrowth to better advantage. It has a cauliflower like appearance. Again, the cortex is intact and there is no apparent soft tissue mass. An MRI was performed, which again showed an osseous outgrowth originating from the posterior right iliac wing. There is a thin surrounding cartilage cap. Imaging findings are typical for an osteochondroma/exostosis. Osteochondromas are frequent imaging findings and really are a developmental abnormality rather than a true tumor. They are composed of cortical and medullary bone with an overlying hyaline cartilage cap and must be continuous with the underlying bone. Osteochondromas can be solitary or multiple. The sessile variety has a broad connection to the underlying bone, whereas the pedunculated variety has a narrow stalk, which points away from the nearest joint. Complications include deformities, adventitial bursa formation, fracture, vascular compromise, neurologic sequelae, and malignant transformation. Malignant transformation is seen in 1% of solitary osteochondromas. Osteochondromas grow during childhood. Continued growth after physeal fusion and a cartilage cap greater than 2 cm, suggests malignant transformation. Symptomatic lesions usually occur in younger patients before the age of 20 years. There is a male predilection. Lymph node or phyllodes tumor? A postmenopausal woman, without complaints, with no family history of disease. An abnormality was detected on the left side on ultrasound. Some colleagues may have missed the abnormality, assuming that the patient had a lymph node on the left. Differential diagnosis was carried out: 🔹 Mammography with tomosynthesis 🔹 Ultrasound-guided biopsy More experienced colleagues were able to identify a rare type of fibroadenoma - phyllodes tumor, despite the typical location of the abnormality for a lymph node. CT e mëvonshme me kontrast tregoi metastaza hepatike hiperpërmirësuese (majat e shigjetave) dhe lezione në mesenterinë e zorrëve të vogla dhe ileumin terminal (nuk shfaqen në rast). Imazhet koronale të fazës arteriale zbuluan një refluks të veçantë të kontrastit në venën kava inferiore hepatike (shigjeta). Simptomat dhe imazhet sugjerojnë tumor neuroendokrin metastatik me insuficiencë dytësore të valvulës trikuspidale. Biopsia e mëlçisë konfirmoi karcinomën neuroendokrine metastatike. ... 🔻Imaging Cancer - Pulsatile Inferior Vena Cava Contrast Material Reflux in Carcinoid Heart Disease 🔻Figure: CT image of a 67-year-old man with a 2-year history of flushing and worsening diarrhea. Physical exam revealed an enlarged liver, and abdominal ultrasound showed multiple liver masses. Subsequent CT with contrast showed hyperenhancing hepatic metastasis (arrowheads) and lesions in the small-bowel mesentery and terminal ileum (not shown). Coronal images of the arterial phase revealed a peculiar reflux of contrast into the hepatic inferior vena cava (arrows). Symptoms and imaging suggested metastatic neuroendocrine tumor with secondary tricuspid valve insufficiency. Është i pranishëm edhe bashkimi kongjenital i C3 dhe C4. 🔻Lezioni shtrihet në foramenin neural të djathtë dhe rrafshon dukshëm kordën spinale, siç shihet në pamjen aksiale me peshë T2. 🔻Vendndodhja e këtij lezioni dhe karakteristikat e sinjalit MR janë shumë sugjeruese për një schwannomë 🔻Pas administrimit të kontrastit, përmirësimi intensiv heterogjen rezulton në një pamje të pazakontë, të buzëqeshur. 🔻Ky imazh ishte shumë argëtues për të mos u ndarë! ... 🔻A Smile in Contrast 🔻A 21 year-old female presents with a 2 month history of neck pain and right arm radiculopathy. 🔻A T1-weighted sagittal image demonstrates an extramedullary, intraspinal mass towards the right at C5. The mass measures 50 mm and has multiple smooth internal septations. Anteriorly and medially, the lesion extends into the lower part of the left side of the oral cavity. After contrast administration, smooth wall and septal enhancement are noted. Adjacent fat planes are normal. No signs of inflammation are seen. 🔻The pathology report from an FNA reported this as a sialocele. 🔻Sialoceles are cystic spaces that arise within the salivary gland. Our patient is a 5 year old girl that was found to have a bilateral genu varus deformity a while back. The mother was offered surgery, but she deferred it until they emigrated to this country. The x-rays show beaking of the medial tibial metaphysis. The epiphysis is quite hypoplastic medially, almost completely non-ossified. The metaphyseal-diaphyseal angle (MDA) is increased to 25 º (swipe). A normal angle measures < 11º. MDA is measured drawing a line between the two beaks of the metaphysis and a line perpendicular to the lateral aspect of the tibial diaphysis (swipe). Blount disease, also known as tibia vara, is caused by disruption of normal cartilage growth at the proximal medial metaphysis of the tibia by excessive compressive forces. Blount disease can be either unilateral or bilateral. Depending on the age of presentation it is classified as infantile and adolescent. The infantile form is commonly bilateral and seen between the ages of 1-5. It tends to worsen with walking. The adolescent form manifests after age 10 and may be unilateral or bilateral. The precise pathogenesis of Blount disease is unknown. Vascular failure during the epiphyseal growth is a presumed cause of the growth disturbance of the proximal medial tibia. Risk factors are obesity, early walking, and African-American heritage. The severity varies from articular cartilage irregularities, premature osteoarthritis to limb length discrepancies. The treatment of Blount disease ranges from bracing to surgical interventions and depends on the age and severity at presentation. Treatment options include proximal tibial osteotomies with either acute or gradual fixation, and hemiepiphysiodesis. The prosencephalon forms the cerebral hemispheres, the thalami, and the basal ganglia. 🔻Alobar holoprosencephaly is the most severe form and shows undifferentiated holosphere of the cerebral parenchyma with a central monoventricle and fused thalami.
