Get good bleed site. Advance Buccal flap with periosteal scoring. Mix of auto,allo,xenograft bone. Isolate with collagen membrane. Sausage technique! Resorba sutures. Periosteal tag. And or tacs. Achieve passive primary closure of margins. Often. Lingual raising of tissue required! Blunt dissection. Dr Pikos technique. Closure. Depending on width anticipated. 6 to 9 months healing. Expose and find good vascularized bone ridge for implant placement. CBCT scan ALWAYS taken prior to surgery. Asses bone volume available , lingual undercut and mandibular nerve position. GBR ( guided bone regeneration) procedures with the use of particulate bone particles combing a mixture of autogenous (patients own bone), allograft ( human donor bone from bone bank) and xenografts ( bone from an animal that provides structural form and durability during bone healing and formation) has allowed bone to be reproduced both horizontally ( as in this case) ie width of ridge and vertically in other cases. Patient referred after I had placed previous implants on failing teeth. 2nd quad ( upper right on panarex X-ray ) and lower teeth taken out. Patient was ok with first molar / bicuspid occlusion ( how bite comes together and meets ). Patient did not want All-onFour tx or all on 6 ( which I prefer to only 4 implants in maxilla) but wanted traditional crown and bridge on implants. An all on ? Would have involved removal of other implants ( which would be ludicrous) to remove bone to allow for prosthetic frame work of a screw retained hybrid bridge ( which is used in all on 4 in mandible and 6 implants in maxilla ! Anyways, the meaning of this post is to see the final development of bone in previous extraction sites in anterior maxilla area ( pre- maxilla) ! We were able to place 3 implants strategically in the 12/11/22 areas four eventual 4 unit screw retained fixed crown and bridge form final prosthesis.13 was retained. 15/14 was removed and implant placed in 15 area. Note the bleeding of fresh bone when exposed.
